Job overview

This is an exciting opportunity to join our expanding palliative support team, delivering excellent palliative and end of life care in patients homes.

A full UK driving license and access to a vehicle are essential for this post. Please note that an international driving license is not accepted.

Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP. 

Main duties of the job

The post holder will work as part of the Community Palliative Care Team and  undertake a broad range of nursing support duties relating to the care of patients  with end of life care needs in the place where they live, usually their own homes. 

The post holder will work under the direction of the Palliative Care Team providing  care as required to patients with palliative and end of life needs. The post holder will  work within the clearly defined boundaries of their competencies.

Detailed job description and main responsibilities

Communication 

  • To have a basic understanding of the psychological and spiritual needs of patients and their families and communicate with them as appropriate, but referring to the Clinical Nurse Specialist or District Nurse as necessary 
  • Establishes and maintains effective communications with patients, carers and health professionals in a professional manner 
  • In line with UHCW documentation policy update patient records ensuring entries are accurate relevant legible and timely and communicates care provided appropriately.

Analytical and Judgemental Skills/Freedom to Act 

  •  To be able to recognise their own clinical limitations and seek help appropriately

Planning and Organisational Skills

  • Maintains IT competency in order to make full use of mobile phones email the internet and on-line training
