Are you an experienced and motivated Occupational Therapist looking to take the next step in your career? We are offering an exciting 12-month fixed term/secondment opportunity to lead within the One Coventry Integrated Team – a pioneering service that brings together health and social care professionals to deliver seamless, person-centred care.

As Occupational Therapy Lead, you will work within the community enablement pathway, supporting the team to deliver specialised occupational therapy to individuals with a wide range of complex needs. The role involves supporting people predominantly in their place of residence, with a focus on reducing unnecessary hospital admissions and enabling safe, effective community-based care.

Secondment will be considered for internal candidates – If you are successful after interview, you must obtain approval from your current line manager to accept the offer.

Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP. 

Main duties of the job

This role offers a unique chance to develop your leadership skills and gain valuable experience across community services. You will be part of an innovative team supporting reablement, hospital discharge, and admission avoidance across urgent and emergency care pathways. Working collaboratively with multidisciplinary colleagues, you will help shape and influence the delivery of integrated care for the people of Coventry.

  • Work as an integral part of the Local Integrated Team (LIT) alongside team leads and LIT manager to ensure high standards of service delivery.
  • Support the team to plan and implement evidence-based, patient-centred treatment in collaboration with patients, carers, multidisciplinary colleagues, and external agencies. Providing clinical support, expertise and guidance as required.
  • Triage, streaming and prioritisation of referrals into the Local Integrated Team. Leading case allocation, liaising with wider system colleagues and external partners, providing professional challenge and advice for routine and urgent cases as required.
  • Chairing weekly multi-disciplinary team meetings with the wider service and external care providers. Support with clinical reasoning, ensuring system flow and highlighting blocks to case progression. 
  • Provide clinical supervision, support, training, and development for occupational therapy staff, as well as wider team development

Detailed job description and main responsibilities

  • Lead on occupational therapy quality improvement, service development, and innovation initiatives.
  • Collaborate with team and service leads to advise on occupational therapy needs and contribute to integrated clinical pathway development.
  • Ensure accurate and timely data entry, maintaining professional standards and continuous professional development.
  • Work closely with adult social care and third-sector providers to deliver safe, effective care.
  • Support OCIT Service Leads with day-to-day service management and contribute to monitoring and evaluation of service effectiveness.
  • Support timely response and investigation of incidents and complaints with the support of the LIT manager, taking forward actions and learning opportunities as required.
