Job overview
An exciting opportunity has arisen for a Patient Flow Coordinator to support the new and transformational One Coventry Integrated Team (OCIT) - a pioneering service that brings together health and social care professionals to deliver seamless, person-centred care. You will be part of an innovative team supporting reablement, hospital discharge, and admission avoidance across urgent and emergency care pathways.
Local integrated care is one of the Trust’s fundamental purposes, as set out in our refreshed strategy for 2022-2030; ‘More than a hospital’. The Trust has an ambition to deliver improvements in the health and wellbeing of our local populations through collaboration with partners across the health and social care system. The Patient Flow Coordinator role is key to the successful delivery of this exciting and transformational service aimed to improve the health and wellbeing of the Coventry population.
The Patient Flow Coordinator will work autonomously within OCIT demonstrating effective time management, coordinating a varied caseload in line with dynamic capacity and demand challenges, whilst advising and supporting the local integrated teams with appropriate service pathways. The successful candidate will have effective communication skills, a high level of administrative skills, an eye for detail with a focus on service coordination and data collection.
Main duties of the job
- To deliver training in pathways and system support tools to support assessments as required ensuring safe discharge planning and patient flow coordination with the correct identified level of support
- To support efficient use of resources and consistent flow for those patients case managed by OCIT.
- To coordinate assessments and referrals for patients accessing the OCIT service considering capacity/demand and prioritisation level with support from clinicians.
- To communicate with patients / families / carers / Care Homes of the patients within OCIT pathways and support coordination and flow, planning meetings as required to support.
- To work independently within the service, seeking senior support as required on tasks outside of identified competency.
- Provide education, support and advice to the team on patient flow, discharge process and pathways.
- To represent Complex Flow Improvement lead where applicable, provide team development and caseload updates to managers and inform LIT manager of system blockages or areas of concern.
- Escalate and report any concerns via LIT manager to highlight delays to assessment and/or discharge.

Detailed job description and main responsibilities
- To establish and maintain effective communication with patients, carers, MDT, other workers and agencies across health, social care and the private sector.
- To resolve issues for patients and staff to ensure safe discharge, patient flow and escalate issues accordingly to LIT manager
- Utilise IT systems to secure accurate, timely patient data. To enter data onto specialised databases for the purposes of audit, within data protection guidelines.
- To have an awareness of national standards in relation to Urgent Community Response and key performance indicators when coordinating patient flow across Urgent Emergency Care pathways
- To have an awareness of the wider OCIT staffing capabilities and coordinate efficient use of staff skill and resource, working with the wider service Patient Flow Co-ordinators as required.
- Communicate all relevant information about the patients within the OCIT caseload to the multi- disciplinary team and ensure accurate recording for handover of patients in your absence.
- Promote and monitor adherence to Health and Safety and Trust policy designed to protect healthcare staff and service users from known hazards.
- Work closely with care homes to facilitate flow to their normal place of residence.
- Maintain a clean, safe environment ensuring adherence to Trust standards of cleanliness, hygiene and infection control at all times.
- To ensure confidentiality of patient and staff information is maintained at all times.
- To promote awareness of the role of OCIT and the Local Integrated Teams.

The group is currently trialling a new Integrated Neighbourhood Team (INT) model following a collaborative event with internal and external stakeholders. This pilot aims to explore team structures and assess the impact on patient experience. While no formal redesign has taken place yet, the outcomes of this trial may inform future service delivery and team configuration across the group.
If you are successful in your application, please be aware that your team structure and line management may be subject to change in the future as part of this evolving model. However, any formal changes will be preceded by appropriate consultation in line with organisational policy.
