Registered Nurse, Integrated Neighbourhood Team

NHS AfC: Band 5

Job overview

Registered Nurse – Integrated Neighbourhood Team (Wound Healing Focus)

An exciting opportunity has arisen for an enthusiastic and motivated Registered Nurse to join the Integrated Neighbourhood Team (INT) at University Hospitals Coventry and Warwickshire NHS Trust.

Working as part of a multidisciplinary neighbourhood team, you will provide high-quality, evidence-based care to patients in their own homes, care homes and community clinic settings. You will be responsible for assessing, planning, delivering and evaluating care for patients with a range of acute, chronic and complex healthcare needs, promoting independence and supporting patients to remain safely within their own communities. A significant element of this role will involve supporting patients with wound healing needs, including lower limb ulceration, pressure ulcers, surgical wounds, lymphovenous conditions and other complex wounds. You will undertake holistic assessments, develop evidence-based treatment plans and evaluate healing outcomes whilst working within agreed clinical pathways. Whilst wound healing will be a priority area of practice, the role extends to the wider management of patients on your caseload. Nurses will be expected to deliver holistic care in line with individual patient needs, including medication management, insulin administration, long-term condition management, clinical monitoring and other community nursing interventions as required.

Main duties of the job

About the Neighbourhood Team

Our Integrated Neighbourhood Teams provide coordinated care closer to home, bringing together health and social care professionals to provide proactive, responsive and personalised care for local populations.

The team supports patients with acute, chronic and complex health needs, helping to prevent avoidable hospital admissions, facilitate hospital discharge and promote independence. We work collaboratively across services to ensure patients receive seamless care that meets their physical, psychological and social needs.

As a Registered Nurse within the Neighbourhood Team, you will play a key role in delivering holistic care, coordinating treatment plans and supporting positive outcomes for patients across a diverse community caseload.

The successful candidate must have post-preceptorship experience

Please note this post is not suitable for applicants who are required to undertake the OSCE training programme. Applicants who do not have current valid UK NMC registration will not be considered for this post.

Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP
