Are you a highly motivated and skilled manager/clinician with management experience and have an interest to develop ?

We are looking for an individual to work as part of a dynamic team in delivering our NHS Continuing Healthcare Services. 

The post holder will manage the core clinical team on the delivery of assessments, ensuring that they meet the key requirements of all KPI’s, assessments are of the quality expected and support staff as required. All HR processes will need to be managed where appropriate for staff, clinical supervision where appropriate and training needs.                                                                                                 

The CHC Nurse Specialist will lead a team to deliver a professional and efficient service in line with the NHS Continuing Healthcare national framework and standards.          

The post holder will have a flexible approach to care and will work across inter-agency boundaries, maintaining good communication and liaison skills with all stakeholders. The post holder will support the Clinical Lead to ensure that their work stream of programmes is planned and managed effectively and take the lead in assisting in their successful delivery.

Main duties of the job

The post holder will line manage a team of Continuing Healthcare Nurse Practitioners responsible for facilitating multi-disciplinary assessments and patient reviews in line with standard operating policy and procedures. The post holder will ensure that they and their team adhere at all times to the relevant National Framework and guidance, NMC Code of Professional Conduct and work within established policies, procedures, protocols and systems ensuring quality standards and performance as directed by the Specialist Nurse/Clinical Manager.

 

The post holder will support the Clinical Manager to ensure that their workstream is planned and managed effectively and take the lead in assisting in their successful delivery.

 To be successful in this position, it is essential that you are:

·         Professionally registered as a Nurse with significant post-registration

·         Experienced in case management and patient assessments;

·         Experienced in working within and supporting a multi-disciplinary team;

·         An excellent communicator with evidence of advanced verbal and written communication skills;

·         Experienced in using tact and diplomacy, with demonstrable problem solving skills;

·         Dedicated to providing high standards of patient car

·         A high level of evidence-based decision-making skills
