Job overview

Cross site role between Hospital of St Cross and University Hospital Coventry.

Provide advance, specialist nursing care for people with diabetes across two clinical sites with a particular focus on insulin pump therapy. The post holder will be part of a larger team responsible for education, complex case management, service development and ensuring high quality, evidence based care with improved patient outcomes.

The role will be largely be outpatient work, with support for in patients who attend on insulin pump therapy.

Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP. 

Main duties of the job

  • Provide specialist care and management for patients with Diabetes.
  • Assess, Plan, Implement and Evaluate diabetes treatment.
  • Educate patients on self management, lifestyle and technologies.
  • Provide training and advice to staff and contribute to service improvement.

Detailed job description and main responsibilities

Key responsibilities:-

  • Deliver expert care for patients with Type 1 and complex Type 2 diabetes, including those using insulin pump therapy.
  • Initiate, adjust, and optimise CSII and multiple daily injection insulin regimens.
  • Education around management of actute diabetes presentations including Diabetic Ketoacidosis and severe hypoglycaemia.
  • Interpret glucose data (CGMS, flash and capillary readings) to guide treatment decisions.

Education and training:-

  • Deliver structured education to patients on:
    -    Insulin pump use
    -    Sick day rules
    -    Carbohydrate counting
  • Assist in the providing train and support for healthcare professionals in diabetes technology.
  • Act as a clinical resource for complex diabetes queries.

Multidisciplinary working:-

  • Work closely with consultants, nurses, and dietitians to commence patients on insulin pump therapy and provide follow up reviews.
  • Work with insulin pump providers to ensure safe use and be able to support patients with new technologies as they are being implemented and upgraded.

Leadership and management:-

  • Support other team members with use of insulin pump therapy as they become more standard care within Type 1 diabetes.
  • Assist in the service co-ordination of insulin pump clinics across the two sites.

Governance and safety

  • Maintain accurate, contemporaneous clinical documentation.
  • Ensure safe practice in line with Trust policies, including insulin safety standards.
  • Participate in incident reporting (DATIX), investigations, and shared learning.
  • Contribute to clinical audits and ensure actions are implemented.

Communication 

  • Work collaboratively within the multidisciplinary team (MDT), including consultants, dietitians, and primary care.
  • Communicate complex clinical information clearly to patients and families.
  • Liaise with external providers, including community teams and device companies.
