Community Mental Health Nurse, MH Homeless Team
NHS AfC: Band 6
Come and join us
We’re looking for people who share our values of compassion, accountability and optimism to help us provide high quality care for the patients, carers, families and communities we serve. We specialise in NHS mental health and learning disability services and we’re proud to make a difference every day.
At SPFT we put people first. We’ll do everything possible to help you feel respected, valued and included. You’ll have opportunities to learn, grow and gain new experiences to support your career, with access to lifelong learning and professional development.
We actively promote flexible working and understand how important work life balance is, especially in healthcare. Whether it’s part time hours, hybrid working or flexible start and finish times, we’ll explore what works for you and the role.
We recognise that tools like AI are becoming part of everyday life and you may choose to use them when preparing your application. We welcome this, as long as what you submit reflects your own experience, skills and voice. AI can be helpful for structuring or refining your answers, but it should not replace your own examples or create content that isn’t based on your real experience.
Our recruitment process is designed to get to know you, and we’ll explore your examples further at interview, so it’s important your application genuinely represents what you bring. You can read more about using AI in applications and interviews on our recruitment process page.
We’re shaping a workplace where ideas are encouraged, technology supports you and everyone has a voice in how we improve. If that sounds like the kind of organisation you want to be part of, we’d love to hear from you.
Job overview
An opportunity has arisen for a Registered Mental Health Nurse to join the Mental Health Homeless Team (MHHT). This is a one-year fixed term contract focused on supporting our colleagues and residents at Phase One Hostel in Brighton.
Main duties of the job
Based in East Brighton Community Mental Health Centre, MHHT is a small multi-disciplinary specialist team providing assessment, treatment and support to homeless or insecurely housed people who have a severe and enduring mental illness and who may have additional substance misuse problems. You will conduct complex assessments and risk assessments in a variety of settings and manage a complex caseload as a lead practitioner.
Phase One is a mixed gender 52 bed high support hostel for single homeless people with complex support needs. The service works with clients who are vulnerable with chaotic lifestyles to increase self-esteem and create a space where they can begin to address the issues at the root cause of their homelessness, make the changes necessary to lead more settled lives, and realise their aspirations and goals. Phase One works with homeless men and women with in a person-centred, trauma-informed way to address issues of substance use, mental health and offending behaviour whilst empowering people to take responsibility and move towards the goal of gaining stability in their lives.
Working for our organisation
The post offers an exciting opportunity to help shape and develop a targeted mental health outreach approach within hostel services. Applicants will need experience of working with people experiencing CoSUM (co-occurring substance use and mental health disorders) and an understanding of the complexities associated with supporting this client group. Individuals with co-occurring mental health and substance use needs frequently experience high levels of unmet need and poorer physical, mental, and social outcomes compared to those living with either condition alone. The successful applicant will therefore need to demonstrate confidence in working collaboratively across services, managing risk, and delivering compassionate, recovery-focused interventions for people with complex and often enduring presentations.
We aim to provide a direct and assertive outreach service, working in partnership with other local homeless agencies from both the statutory and voluntary sector. You will require a basic understanding of housing legislation as it relates to Brighton and Hove and be familiar with recent changes in social legislation as it relates to this client group.
