Job overview

We are looking for an enthusiastic and committed Community Support Worker to join our Intensive Support Team based at St Anne's Centre, St Leonards on Sea support our Hastings & Rother Neighbourhood Teams.

The team works intensively with clients for up to eight weeks to reduce risk of admission and to support post discharge from mental health units. The team provides a liaison role and is in regular contact with inpatient services to facilitate discharge.

You will be providing assessment and treatment intervention both to older adults with age-related complex mental health needs and to adults of any age with dementia.

Hours to be worked across the week are from 08:30 until 19:00 during the week and 09:00 to 17:00 at weekends.  Some weekend shifts will be required.

The post holder must be able to drive.

Main duties of the job

The main duties include

Promote the independence of  service users within the community setting.

To provide regular and practical support to service users.

To help service users gain access to resources to include benefits and welfare rights.

To help service users access community supports such as day centres, clubs and day services.

To ensure that the care plan is fully implemented by the service user in accordance with service specifications including liaising with and providing information to other agencies involved in the care of the service user.

Monitor the quality of care delivered and service user and carer satisfaction with the service.

Assist in reviews of service user in the community on a systematic basis so as to ensure the continued need and appropriateness of services provided by the Trust and maintain review records.

Adhere to, and where appropriate, ensure the implementation of relevant legislation, Department and Trust policies and procedures.

Maintain accurate and up-to-date records as required by the Trust.

Participate in a formal programme of supervision.
