Job overview
As a Case Manager, the post holder will work as part of the Provider Collaborative team in delivering an effective service to ensure safe and effective care pathways are in place for Child and Adolescent service users from the Hampshire, Sussex and Kent region in mental health care settings within the NHS and Independent sector.
The role is to hold and manage a defined caseload of young people in tier 4 CAMHS services which includes hospitals and alternatives to admission. The role includes supporting families and carers of those young people accessing Tier 4 services.
To provide clinical support to the Provider Collaborative in relation to the defined population, this will include projected mapping of future service use, identification of users for future service provision, planning in terms of future service needs.
The role supports the business in driving transformation as well as value for money in planning, commissioning and service the role is designed to build a combination of subject matter expertise and technical skills to develop a strong service delivery.
Travel to see young people is vital. The base is negotiable across the Hampshire, Sussex and Kent footprint.
Main duties of the job
This is an exciting opportunity to join the Hampshire, Sussex and Kent CAMHS Provider Collaborative as an inpatient case manager, supporting effective patient pathways across the footprint.
Case managers play a vital role within Provider Collaboratives, providing credible oversight and facilitator of care for patients in receipt of specialised services. They also support the commissioning function by assisting with the monitoring and reviewing of the quality of provision by observing providers in practice and monitoring and reviewing quality information and local intelligence.

Detailed job description and main responsibilities
- Provision of clinical expertise to the Hampshire, Sussex and Kent and Provider Collaborative.
- Ensuring effective care pathways, within and across Provider Collaborative Boundaries.
- Resolution of complex or disrupted care pathways in conjunction with colleagues across England (and within UK for service users placed in services outside England)
- Supporting the Provider Collaborative team in ensuring that services within the footprint are compliant to all specifications and
- Ensuring the delivery of high-quality services that are value for money
- Ensuring case management systems and record keeping are maintained and kept up to date
- Maintaining a caseload of young people and supporting their families/carers
- The person will be required to work as duty within office hours with admin support as part of a rota system with other case managers
- Attending clinical meetings to support alternative to admissions

Qualifications
Essential criteria
- Registered Mental Health/Learning Disability Nurse or other registered health or social care professional
- Possess a relevant additional professional qualification e.g., masters level, or equivalent level of experience of working at a senior level in specialist area
Knowledge/experience
Essential criteria
- Detailed knowledge of legal issues pertaining to the specialist service including MHA 1983, Children's Act, S117 after care, Transforming care, Safeguarding and child protection legislation
- Detailed operational knowledge of specialised services, including risk assessment/management
- Extensive knowledge of specialist areas, acquired through post graduate diploma or equivalent experience or training plus further specialist knowledge or experience to masters level equivalent.
- Evidence of post qualifying and continuing professional development
- Must have an understanding of the background to and aims of current healthcare policy and appreciate the implications of this engagement
