Are you a passionate, and caring district nurse, who can oversee the provision of high-quality holistic care for people in their own home with acute, complex, and long-term conditions?

We are seeking an experienced and highly motivated District Nurse to join our community nursing service, within Planned Care. This is a key leadership role, providing expert clinical care and visible management to support patients with increasingly complex health needs within their own homes and community settings. The post holder will be responsible for leading and developing a dedicated team of community nurses and healthcare support workers, ensuring safe, high-quality, and compassionate care is delivered at all times.

As a District Nurse, you will provide advanced assessment, care planning, and case management, working in partnership with patients, families, carers, GPs, and wider multi-disciplinary teams. You will play a vital role in supporting the prevention of hospital admissions, facilitating timely discharges, and enabling people to remain independent for as long as possible.

Main duties of the job

The District Nurse Caseload Manager is a predominantly Clinical role, time will be split with an 80% Clinical and 20% Non-clinical focus.

The Caseload Manager will be responsible for the day-to-day smooth running of the caseload and hold clinical responsibility for the caseload they are managing and leading on, ensuring patients are appropriately assessed and care is delivered in a timely manner with the involvement of patients and carers as appropriate.

The caseload holder will also ensure that team members are appropriately trained, supported and, have the right knowledge, skills, and competencies to meet the requirements of the role.

  • District Nursing Specialist Community Practitioner Qualification (DNSPQ)
  • Holds V300 qualification (committed to working towards)
Desirable criteria
  • Specific CPD modules relevant to specialist field
  • Leadership and management qualification

Experience

  • Experience in the specialist clinical field District Nursing
  • Experience in the teaching and training of others in a variety of settings e.g. clinical, academic
