*Must have a professional qualification in mental health nursing, social work or occupational therapy.

We are pleased to offer the new role of a Care Coordinator in the Future Focus team. The role will involve primary responsibility for the care coordination of a group of service users with an ‘At Risk Mental State’ or who are at ‘Ultra High Risk’ of psychosis. The service spans the age ranges of 14-35, with this post primarily working within the adult age range and service, although there will be an expectation to work some of the time within the CAMHS element of the service to support service-users aged under 18 if required (depending on the post holders caseload capacity). The role will involve assessment, formulation and signposting onto appropriate services, alongside planning and providing packages of care for individuals and their families when they are experiencing mental health difficulties including attenuated psychotic symptoms and other complex needs. There will also be close working with the wider team, which includes psychological therapists, recovery coordinators and support workers.

In our efforts to develop a representative workforce and develop partnerships with the communities of Bradford, we would like to encourage applications from individuals of a Black Asian and Minority Ethnic (BAME) background who may be representative of the diverse community in Bradford.
