Job overview

We are seeking a motivated and compassionate Band 6 Community Care Coordinator to join our Community Mental Health Team in Bradford. This is an exciting opportunity to make a real difference in the lives of service users by delivering high-quality, person-centred mental health care.

As a Care Coordinator, you will manage a defined caseload, working autonomously and collaboratively within a multidisciplinary team. You will be responsible for assessment, planning, intervention, and evaluation of care, ensuring that service users receive safe, effective, and recovery-focused support.

Main duties of the job

Key Responsibilities:

  • Coordinate and deliver community mental health care in line with Trust policies.
  • Undertake comprehensive assessments and manage risk effectively.
  • Work collaboratively with service users, carers, and partner agencies.
  • Administer and monitor medication in accordance with NMC and Trust guidelines.
  • Participate in case conferences and contribute to service development.
  • Promote equality, diversity, and safeguarding in all aspects of care.

Essential criteria
  • Professional qualification in mental health nursing, social work, occupational therapy
  • Degree level education or equivalent experience
  • Registration with Professional Body (e.g. NMC, SWE, HCPC)

Knowledge

  • Experience of working in a community mental health setting
