Are you a passionate and motivated nurse looking to make a real difference in the lives of children and young people with Special Educational Needs and Disabilities (SEND) and complex health Needs? We have an exciting opportunity for a Registered Nurse (Band 5) to join our dedicated School Nursing Special Needs Team (SNSN) at Bradford District Care Foundation Trust on a 12-month fixed-term basis.

The SNSN Team is a friendly and child-centered service supporting children and young people attending special schools across Bradford, Keighley, and Brooklands School in Craven. We are an evolving service, committed to ensuring that all children have equal access to high-quality, holistic care within their educational setting.
This is a mobile role, offering flexible working across home, office, and school locations.

Main duties of the job
Key Responsibilities
-Undertake initial health assessments and contribute to care planning
-Deliver training to school staff and health promotion activities
-Work effectively within multi-disciplinary teams
-Participate in safeguarding procedures and care coordination
-Support the immunisation programme in special schools
-Contribute to service development and engage with children and families through social media and digital platforms
To work as a Staff Nurse within the School Nursing Special Needs team, to provide a safe and effective service for school age children in special schools and the community, in the Bradford, Airedale and Craven Districts, that promotes the physical, social and emotional wellbeing of the child.
The School Nursing Special Needs Team support children and young people from 4-19 years of age to access school safely, who are in a Special Needs school and have complex health and behavioural needs.
To work as a Staff Nurse within the School Nursing Special Needs Service which includes: completing initial health assessments with the family before a child starts school.
To provide school staff training to support children and young people accessing these services.
Writing and updating care plans for students who require them and liaise with other professionals.
To ensure care is clinically effective and of a high standard.
To deliver immunisation campaigns.
To promote the physical, social and emotional wellbeing of the child.
