Job overview

The Children’s Outpatient Service is delivered across 2 hospital sites and one community clinic. We support children and young people aged 0-16 years with a variety of conditions. We run general paediatric clinics as well as specialist clinics such as Diabetes, Asthma and Coeliac. We also have visiting Consultants from regional centres who attend our clinics to offer joint specialised clinics, these include cardiology, genetics and surgery.

We deliver a dedicated Paediatric Phlebotomy and allergy testing services. We offer a wide range of development opportunities including, leadership and management training, skills training, service development opportunities.

We are focused on improving our patients and families experience and ensuring they receive the best, compassionate, individualised care throughout their health care journey. We go the ‘extra mile’ for all patients and their family, and appreciate our responsibility for being the ‘window’ to the NHS for some children.

 

 

Main duties of the job

To be responsible for the assessment, planning, implementation and evaluation of evidence based individualised plan of care. The post holder will also assist in the management and organisation of nursing work in the department

- To be responsible for providing the appropriate nursing evidence based care of an allocated group of patients.

- To work as part of a multidisciplinary team using resources efficiently.

- To deliver quality care to patients and to supervise the working of junior staff during the span of duty.

- To be competent in carrying out a variety of clinical procedures.

Working for our organisation

The Children's Outpatients Nursing Team is a relatively small team consisting of Children's Nurses, Health Care Assistants and Administrative support. We work closely with extended members of the multidisciplinary team.

Our team belongs to the Children's Directorate within the Family and Specialist Services Division at Calderdale and Huddersfield NHS Foundation Trust.

Detailed job description and main responsibilities

Clinical Duties 

- To contribute to a culture that recognises and works in partnership with patients and their families, involving them in all decisions.

- To participate in the assessing, planning, implementation and evaluation of individualised plan of care.

- To have a willingness to take on extended roles i.e. IV administration/cannulation/venepuncture, care of central lines, APLS, food challenges and other clinical roles linked to the department.

- To work within the NMC code of professional practice, and within policies, procedures and guidelines of Calderdale & Huddersfield NHS Trust.

- To ensure that high standards of nursing care are given and maintained.

- To act when standards are not being maintained

- To recognise changes in a patients’ condition which require the intervention of others and refer on as appropriate

- To act as an advocate for the patient and the family, including promoting and protecting their interests, rights equality and inclusion and privacy

- To act as a ‘Link Nurse’ within a defined speciality/ interest area

- To be involved in trust Initiatives to improve patient safety and care, e.g. Exemplar ward, Safer Patient Initiative.

Professional Duties

- To be aware of and adhere to Trust Policies relating to the control and administration of drugs.

- To ensure that high standards of nursing care are given and maintained. And to act when standards are not being maintained.

- To work within the NMC code of professional practice, and within policies, procedures and guidelines of Calderdale & Huddersfield Foundation Trust.

- To participate in regular staff meetings

- To be flexible to work within areas or departments to meet the demands of changing service needs.

- To provide mentorship and preceptorship to newly qualified staff and students, new starters and students encouraging an effective learning environment.

- To take responsibility for own continuing professional development and performance, including identifying own development needs and maintain own portfolio in accordance with re-registration requirements

Managerial Duties

- To communicate with the nurse in charge and collect data in order to participate in the smooth running and organisation of the department.

- To support junior and senior colleagues in their roles and be resilient to manage change.

- To support the nursing sister in risk assessment and minimisation ensuring incidents and near misses are reported.

- To manage verbal complaints, inform the nursing sister and refer on when unable to resolve.

- To participate in the investigation of incidents / complaints as required.

- To establish and maintain effective communication, both verbal and written and relationships with multi-disciplinary team

- To recognise, prevent where possible, and manage areas of conflict, reporting to nursing sister if unable to resolve.

- To assist in the completion of junior staff’s PDR’s.

- Participate in research programmes utilising agreed research findings to enhance the patient and their family’s experience 

- Actively participate in benchmarking clinical practice along with other areas.

- Contribute to and participate in programmes of Audit, including utilisation of results in practice

- Support the nursing sister in aspects of Patient Experience work.

Financial Duties

- To assist in the promotion and encouragement of innovative thinking in the current climate to ensure the best uses of resources.

- To use and safeguard equipment in a competent manner and ensure it is in good working order.

- To be flexible in working pattern to help meet the needs of the service.

- To support the nursing sister in the management of the budget and resources.

Person specification

QUALIFICATIONS / TRAINING

Essential criteria
  • NMC Registered Nurse in relevant field of practice (or within 6 months of completing a validated BSc or MSc course, resulting in NMC registration)
  • Practice Assessor/Practice Supervisor qualification or willingness to complete
Desirable criteria
  • Additional post-registration qualification relevant to field of practice
  • Evidence of continuing professional development
  • Role Specific Training relevant to specialty of the post
  • Teaching Qualification
  • Leadership Qualification

KNOWLEDGE, EXPERIENCE & EXPERTISE

Essential criteria
  • Knowledge and skill to plan, deliver, and evaluate high quality, evidence-based nursing care to a group of patients within the speciality of the post
  • Experience of working with patients, and those that matter to them, from a range of diverse cultures and backgrounds
  • Experience of working as part of a team, with the ability to prioritise own workload.
  • Knowledge and skill in infection prevention and control principles and procedures
  • Knowledge and skill in safeguarding principles and procedures
  • IT literacy and ability to use NHS systems
  • Competence in speciality or role specific extended skills required of the post or willingness to undertake necessary training.
Desirable criteria
  • Clinical experience in the specialty of the post
  • Skills in research and audit methodology and application of findings to practice
  • Evidence of introducing change and innovation into nursing practice
