Job overview

The role of Discharge Facilitator has been developed to specifically work with inpatient and community colleagues as well as family members/carers and service providers to support smooth discharge pathways out of hospital. 

Using  a multi-disciplinary oriented approach the post holder will be expected to support individuals to access services in pathways 0 to 3 that meet their needs and facilitate timely discharge from hospital.

Main duties of the job

·               You will communicate effectively, sometime complex/sensitive information with all members of the MDT, Social Services, Community services to ensure a smooth well planned and timely discharge for patients.

·               Co-ordinate and provide effective communication of information between hospital and community services to promote collaborative working and ensure a smooth transition for patients and carers.

·               Assess and evaluate rapid discharge patients in a timely manner, discussing with multi professionals’ team.
