Job overview
The Anticoagulant Clinical Nurse Specialist will be responsible for providing a Nurse/Pharmacist Led Anticoagulation Service offering advice and expertise within the area of Anticoagulation and also providing a dosing service for all oral anticoagulants for outpatients. It is expected that once competencies in anticoagulation have been gained that the post holder will undertake the NMP course.
Other duties will include: discharge planning, an outpatient DVT service, management of obstetric patients requiring anticoagulation, perioperative planning for anticoagulated patients and new patient clinics.
They will provide advice on all areas of anticoagulation therapy to colleagues and clients.
Main duties of the job
- To provide a practitioner-led Anticoagulation Service working within the Integrated Care and Community Directorate team working autonomously making independent clinical decisions (a Training program to reflect the achievements required will be commenced with the expectation that all competencies are signed off within 6 months)
- To work within and promote Trust protocols for Anticoagulation
- To run daily weekday outpatient anticoagulant clinics utilising point of care testing device
- Counselling and education of new anticoagulant patients
- To provide a telephone advisory service to answer patients’, medical staff and primary healthcare providers’ enquiries regarding anticoagulation
- To assist in the running of daily weekday outpatient postal clinics accurately adjusting warfarin doses according to protocol with the support of the specialised computer database (this requires working independently and also requires a high level of IT competency)
- To assist in the treatment of over-anticoagulated patients in line with established protocols
- To work to relevant PGDs relating to anticoagulation until NMP course achieved

Detailed job description and main responsibilities

- To collate and store information and statistics for all anticoagulated patients
- To maintain accurate records in line with Trust policy and NMC records and record keeping guidelines
- To work within and promote Trust protocols for Anticoagulation
- To run daily weekday outpatient anticoagulant clinics utilising point of care testing device
- Counselling and education of new anticoagulant patients
- To provide a telephone advisory service to answer patients’, medical staff and primary healthcare providers’ enquiries regarding anticoagulation
- To assist in the running of daily weekday outpatient postal clinics accurately adjusting warfarin doses according to protocol with the support of the specialised computer database (this requires working independently and also requires a high level of IT competency)

- Together with team members ensure accurate maintenance of the Anticoagulation computer database
- To dose adjust oral anticoagulation for inpatients within SwICC
- To liaise with medical and nursing teams regarding anticoagulant related clinical problems with inpatients
- To support and facilitate early discharge of anticoagulant inpatients to maximize patient flow
- Together with team members run the Outpatient DVT clinic facilitating outpatient management, teaching patients/carers to administer low molecular weight heparin injections if appropriate
- In collaboration with the Obstetric Department initiate and monitor anticoagulation for obstetric patients under the direction of the Consultants Haematologists and Obstetricians according to protocol
- To actively participate in the arrangements pre-operative surgical patients receiving anticoagulation within established protocols providing advice to Day Surgery and other areas undertaking surgical procedures
- To comply with and support the Trust complaints procedures
- To assist in research and clinical trials as approved by the Trust
- To communicate urgent results and give appropriate advice to patients and GPs according to protocol
- To work in a professional manner with external agencies to promote good working relationships
- To work within a multidisciplinary team and have an understanding of the role of each member within the team
Person specification
Education, Qualifications and Training
Essential criteria
- Accredited Anticoagulant Course
- NMP or willingness to undertake as soon as sufficient anticoagulant experience has been gained (normally after 12 months)
- PACR or similar course
Desirable criteria
- Completion of any anticoagulation related course
- PACR course or similar
- NMP
Experience
Essential criteria
- Experience of anticoagulation in course of work
- Experience of caring for patients receiving anticoagulation
- In depth experience of leading a team or holding a managerial position
- In depth experience of working under own initiative and capable of critical decision making
Desirable criteria
- Experience with anticoagulant therapy including warfarin and the DOACs
- Experience of running nurse lead clinics
