Job overview
The Critical Care Unit at the Great Western Hospital is pleased to announce the creation of our CESR Fellowship in Intensive Care Medicine. This post has been designed to support a motivated candidate in completing a Portfolio pathway application for a Certificate of Eligibility for Specialist Registration (CESR). The post will initially be for a period of 1 year, starting from August 2025, with extension based on annual appraisal and progression towards CESR.
The post will be tailored to the successful candidate in line with the FICM specialty specific guidance for achieving entry onto the Specialist Register via the Portfolio pathway in Intensive Care Medicine. In addition to general ICM on the Critical Care Unit at the Great Western Hospital we have an arrangement with our local tertiary referral centre at the John Radcliffe Hospital at the Oxford University Hospitals NHS Foundation Trust to offer placements within Neuro ICM, Cardiothoracic ICM and Paediatric ICM. Following discussion with your supervisor we will be able to support the special skills year in a selection of topics. This provides a unique opportunity to train in Intensive Care Medicine with guidance and supervision from those with an appreciation and understanding of the Portfolio pathway.
Main duties of the job
The CESR Fellow in Intensive Care Medicine will play a vital role in delivering high-quality care to critically ill patients under consultant supervision. Their primary clinical duties will include assessing and managing critically ill patients in the ICU, as well as responding to referrals from the wards and the emergency department as part of the ICU referral service. They will participate in daily ward rounds, conduct comprehensive clinical assessments, and formulate management plans for patients requiring advanced organ support. The role also involves active engagement in multidisciplinary team meetings, responding to emergencies, and ensuring safe and effective patient handovers. The fellow will oversee junior doctors and nursing staff, contributing to their training while working towards their CESR portfolio through structured weekly educational sessions, clinical audits, and quality improvement initiatives. Additionally, they will have opportunities to engage in ongoing research.
The programme duration ranges from 12 months to five years, depending on individual requirements, with annual progression based on performance. The role offers the opportunity to develop and refine procedural skills, including advanced airway management, vascular access, bronchoscopy, and Focused Intensive Care Echocardiography (FICE). Fellows will also be encouraged to pursue specialist interests tailored in collaboration with their educational supervisor and the employing department.
In-hours duties
You will be part of the multi-disciplinary team providing care, support and rehabilitation of the critically ill patients admitted to the Critical Care Unit. This involves daily review of all patients followed by a consultant led ward round. You will be part of the medical emergency team (MET) responding to emergencies around the hospital. You will take referrals from the Emergency Department and around the hospital and review patients with the support of the nurse-led Outreach team. You will be responsible for the admission and management of patients admitted to the Critical Care Unit, with support from the consultant.
With the level of experience that the candidate brings to the job, additional responsibilities and opportunities can be explored. Leading ward rounds, carrying out procedures, including diagnostic echocardiogram and lung ultrasound, can all be supported.
Out-of-hours duties
You will participate in the full shift rota whilst in Critical Care. You will receive additional payments for your out-of-hours and additional night on-call hours. The on-call arrangement is on a rolling rota with a 1:6 frequency of nights and days.
With satisfactory progression the role will develop to increase the level of responsibility to mirror that of the consultant. This will be reflected in the rota with added time for personal projects and management responsibilities and a rota incorporating non-resident on-calls.
Clinical Governance
The post-holder will participate in clinical audit, clinical effectiveness, risk management, quality improvement, and any other clinical governance activities as required by the Trust, Health Authorities, and external accrediting bodies.
Personal and Professional Development
The post-holder will be required to keep themself fully up-to-date with their relevant area of practice. Professional or study leave will be granted at the discretion of the Trust, in line with the prevailing Terms and Conditions of Service, to support appropriate study, postgraduate training activities, relevant CME courses and other appropriate personal development needs.
Management
The post-holder will be required to work within the Trust's management policies and procedures, both statutory and internal, accepting that the resources available to the Trust are finite and that all changes in clinical practice or workload, or developments requiring additional resources must have prior agreement with the Trust. They will undertake the administrative duties associated with the care of their patients, and the running of their clinical department under the direction of the Clinical Director.

Essential criteria
- Fully registered with the GMC
- Eligible to work in the UK
- Has achieved MBBS or equivalent medical qualification
- Primary FRCA or MRCP (UK) or MRCEM by time of offer

- Able to prioritise clinical need and manage time effectively.

- Evidence that present career progression is consistent with personal circumstances and that present achievements / competencies correspond with period since gaining primary medical qualification.
- Valid advanced life support (ALS) provider status.
Desirable criteria

Experience

- Minimum 6 months experience in anaesthetics or equivalent airway skills (such as attaining RCoA IAC).
- Minimum 6 months experience in intensive care medicine.
- Experience of inserting central vascular catheters.
- Experience of arterial line insertion.
Desirable criteria
- Experience of working in acute internal medicine
- Wider experience of airway management.
