Job overview

An exciting opportunity has arisen for an enthusiastic and motivated Band 6 Deputy Clinical Flow Manager to join the multi-disciplinary Home First Team based in Wrexham. Now is a great time to join the Home First Team, as we further expand and develop the service to allow more and more patients to be cared for at home.

The post is 37.5 hours covering 7 days a week between 8am and 8pm, to meet the service requirements.

Main duties of the job

The successful candidate will deputise for the Clinical Flow Manager for Community Services and support the Home First Team Leader.

They will also be responsible for the ongoing screening of patients to facilitate safe and timely discharge from hospital through the Discharge to Assess model of care.

The successful post holder will work within a supportive multi-disciplinary team to promote the Home First ethos through ongoing therapeutic interventions in the patient’s own environment. The successful candidate will act as a link between the acute and community services and will work alongside our established Community Resource Team’s in both Wrexham and Flintshire. You will be actively involved in the future development of the service. If you are looking for a new challenge, please do not hesitate to get in touch.

Qualifications and/or Knowledge

Essential criteria
  • 1st Level Nurse NMC Registration
  • Evidence of a Degree level qualification or working towards this within an agreed time frame
  • Evidence of CPD
  • Holds or is actively working towards a relevant Post-graduate Diploma, Diploma to be completed within an agreed timeframe
Desirable criteria
  • Management qualification and /or management experience
  • Mentor/supervisor/assessor

Experience

Essential criteria
  • Significant experience of working either within the clinical speciality or a relevant nursing role
  • Ability to demonstrate application of improvement methodology
Desirable criteria
  • Experience of working in a multidisciplinary team setting
  • Experience/knowledge of senior nurse role

Aptitude and Abilities

  • Ability to communicate and receive complex information
  • The ability to diagnose a problem and take corrective action
