Job overview

An exciting opportunity has arisen for an enthusiastic and motivated Band 5 Registered Nurse to join the multi-disciplinary Home First Team based in Wrexham. The postholder be responsible for the ongoing screening of patients and the identification of care availability to facilitate safe and timely discharge from hospital and provide ongoing support to any ongoing or complex needs within the community through the Discharge to Assess model of care. The successful postholder will work within a supportive multi-disciplinary team to promote the Home First ethos through ongoing therapeutic interventions in the patient’s own environment and on the Ready for Home wards.

The post is 37.5 hours covering 7 days a week between 8am and 6pm, to meet the service requirements.

Main duties of the job

The successful candidate will act as a link between the acute and community services and will work alongside our established Community Resource Team’s in both Wrexham and Flintshire. You will be actively involved in the future development of the service. If you are looking for a new challenge, please do not hesitate to get in touch.
