An opportunity has arisen for an enthusiastic Band 5 Registered Nurse to join our Community Resources team in Wrexham working 24 hours per week.  The post is to support improved service provision, to reduce unnecessary patient hospital admissions and support early discharge home from Hospital.

The hours of work are variable and will include weekend working. The geographical area of cover is across Wrexham.

Full induction into this post will be supported, with opportunities for further career development.

Main duties of the job

The role of the Registered Nurse in this setting is to work across both acute and community services, to support the early discharge and admission avoidance of patients who can be supported at home with a range of community services, focusing on reablement and patient stability.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
