Social Prescribing Link Workers
Hours: x3 posts – 2 x 37 hours per week, 1 x 30 hours per week
Salary: £20,520 per annum pro rata, AUNN Band D
Tenure: Full time roles – permanent; part time role fixed term for approx. 8 months
Based: Community based within PCN’s in NNE & QMC Emergency Department
The Social Prescribing Link Worker Service takes a holistic approach to an individual’s health and wellbeing, connecting people to community groups and statutory services for practical and emotional support. In this position, you will help individuals to improve their health, enhance their wellbeing and maintain independence. We support and enable individuals to identify and develop self-management strategies, with empowerment being at the core of any interventions.
Personal and Family Support Worker
Salary: £25,000 per annum
Location: Odiham, Hampshire with flexibility to work remotely

About the role
This is a unique opportunity for an empathic person to join our team and contribute to the further development of the service. You will have the opportunity to develop and add to your existing skills and knowledge base in the Social Care field whilst providing welfare support to all serving RAF personnel, their immediate family and dependents. You will be conducting need assessments for our service users to plan and create the appropriate intervention required. You’ll work closely with Station Commanding Officers across a vast geographical area, as well as with relevant Local Authorities, Health and other civilian agencies as well as other military organisations and charities. This is an exciting and sometimes unpredictable role where no two days are the same!
