Job overview

We have an exciting opportunity within our community inpatient unit, at Wokingham Community Hospital for a clinical administrator, called a Patient Flow Co-ordinator. The post holder will be an integral member of the multi professional team and will support the patient flow through the admission and discharge process.   

There are two wards within its Inpatient Unit, caring for adults of any age with a wide mix of health care needs. A proportion of the patients will require continued support after discharge, and the post holder is the link between the patient and relatives, the team and the ongoing services required on discharge.

The post holder will attend MDT and allocation meetings ensuring that all referrals are placed in a timely fashion, maintaining a seamless referral process and communicate with patientsrelatives, and external services. Also, the post holder will assist in the coordination of care for safe transfers when a patient is referred into the service from acute hospitals or their homes.  

Main duties of the job

The Patient Flow Co-ordinator will assist in facilitating admissions and dischargesreviewing and processing referral documents and communicating with external services, like the In Reach Team, Intermediate Care, Community Health and Social Services   

The Patient Flow Co-ordinator is an administrator, and a part of the Integrated Therapy/Flow Teams, where clinical knowledge is beneficial from NHS, general practitioners, or social service backgrounds. They will be the point of contact for our service for our patients, relatives and professionals

Education / qualifications

Essential criteria
  • Good general education including GCSE English and Maths (or equivalent)
  • NVQ4 or NVQ3 or recognised certificate in office administration or equivalent experience
  • Evidence of continued role development

Previous experience

Essential criteria
  • Experience of providing administrative support in a health or social care environment at Band 3 (or equivalent
  • Experience of multi-professional working
  • Experience in coordinating a patient workload
  • Experience of providing confidential administrative support to a complex and varied service
  • Competency and experience with filing, information recording and other office management systems
  • Experience of dealing with people who are anxious/distressed/angry

Knowledge, skills and abilities

Essential criteria
  • Have a sound understanding of NHS-wide initiatives and experience of responding positively to ensure local implementation.
  • Ability to provide constructive opinions and views on ideas for service improvement
  • A proven record of being able to work within set timeframes working to priorities and deadlines.

  • Flexible approach to the planning and co-ordination of patient pathways to ensure maximum use of clinical time.
