Job overview
The admission avoidance team want to recruit a Band 7 Advanced Nurse Practitioner with a passion for caring people in their own home and prompting independence through collaborative working. You will need to be able to identify and manage deterioration, work across inpatient and community teams and have excellent organizational skills. The Intermediate Care team is passionate about training and education and the successful candidate will be expected to provide teaching on a regular basis and will have the opportunity to receive in-house training and complete CPD courses as appropriate.
Main duties of the job
The Advanced Nurse Practitioner will hold a caseload and will manage follow up and undertake initial assessment on patients in the community. They will work closely with the MDT team to ensure safe and effective practice and that patients are seen within the required time frame.
- Demonstrate critical thinking and acting within professional boundaries to provide care for the patient, starting with initial history taking and continuing through clinical assessment, diagnosis, treatment and evaluation of care.
- Keep people who are using our services as safe as possible through the use of sound clinical skills and effective risk assessments.
- Work autonomously in making decision based on assessment, diagnosis and interpretation of test results.
- Ensure the best possible clinical outcomes by using up-to-date skills and adhering to evidence based practice and relevant policies and procedures.
- Ensure the people using our services have a good experience by respecting, empowering and working in partnership with people throughout the care planning process.

Qualifications and Training
Essential criteria
- Registered General Nurse, Adult Branch and current NMC Registration
- Advanced physical Assessment with history taking (level 7)
- Commitment to complete Independent Nurse Prescriber course
- Experience and evidence of teaching and assessing e.g. Preceptorship/Mentor
- Must have a full UK driving license and access to a car
Desirable criteria
- Accredited Advanced Practitioner
- Non-Medical Prescribing (level 7)
Experience
Essential criteria
- Experience of reviewing deteriorating patients
- Experience of providing patient facing interventions
- Able to interpret results both biochemistry and near side Experience of leading on recruitment
- Working with a Nursing, Physiotherapy and Occupational therapy team within an inpatient and or community setting
- Experience of providing clinical information to wider MDT
