Are you looking for an exciting change of role in an established but developing service? We are looking to recruit  two dynamic and innovative Band 5 Flow Coordinators to join the Integrated Community Response Service- a multidisciplinary team made up of GPs, an Advanced Clinical Practitioner, occupational therapists, a social worker, physiotherapists, nurses, a community matron, and support workers.  The main aim of the service is to manage complex clinical cases at home, prevent unnecessary hospital admission and support early discharge of patients following an acute admission.

The team provides immediate assessment, intervention and management to a broad range of patients with complex needs. All members of the team work very closely together and have a good relationship with other community teams and clinicians such as district nurses, GPs, London Ambulance Service and mental health services.

We are looking for two enthusiastic and motivated candidates with experience of managing complex discharge pathways and experience of working with multiple providers to provide co-ordination and leadership in line with agreed service objectives.  The successful candidates will be required to contribute to the service staff roster from 8am to 8pm; seven days per week on a shift basis. The shift patterns available are either 12-hour shifts  (8am to 8pm) or a combination of early and late shifts (early = 8am to 6pm and lates = 10am to 8pm)

The postholders will work jointly with the Hounslow Integrated Discharge Hub, hospital teams and the London Borough of Hounslow to ensure patients are effectively supported through these pathways.

As the service is community based, you will ideally need to be a car driver or be able to access the community via bike or public transport.
