Job overview
An exciting opportunity has arisen for an experienced senior Cardiac Rehabilitation Nurse to join our Cardiac Health and Rehabilitation Service.
The post holder will be responsible for the Cardiac Rehabilitation Services across the Charing Cross Hospital, SMH and Hammersmith Hospital as part of the Imperial Healthcare Cardiovascular Health Services. The Service provides care to cardiac patients who have been discharged from hospital including patients with cardiac disease, heart failure as well as those with ICDs.
Imperial Healthcare NHS Trust, the UK’s first academic health sciences centre, provides an internationally renowned tertiary cardiology and cardiothoracic clinical service and has a strong track record in both clinical and basic sciences research.
For this challenging and fulfilling post we are looking to recruit an enthusiastic individual, with staff management experience, with a sound knowledge and understanding of the prevention and management of cardiovascular disease
You will be a Registered Nurse with a wealth of experience in Cardiac Rehabilitation and Cardiology at a senior level. You will be responsible for managing the multidisciplinary team as well as managing own caseload of cardiac patients. You will be championing and delivering the BACPR Core Components and Standards. You will be self-motivated and possess good organisational, communication, and leadership skills.
Main duties of the job
- Deliver advanced nursing care to complex patients, ensuring a holistic approach that prioritises safety, dignity and personal choice.
- Conduct comprehensive assessment, develop, implement and evaluate plans of care, integrating the latest evidence- based practices.
- Provide expert clinical advice, recognising trends in patient outcomes and proactively addressing potential issues.
- Promote patient centred care and advocacy, ensuring the patient and their families are engaged in the decision making process.
- Clinical Responsibility for Patient Care.
- Act as clinical expert in the management of patients with Coronary Disease, demonstrating clinical competence and expertise in the Clinical Nurse Specialist services of prevention, health and cardiac rehabilitation.
- Be able to demonstrate a sound understanding of cardiac symptom management.
- Develop, lead and facilitate programmes of care for patients with Coronary Heart Disease including advice and support to patients and their relatives/carers.
- Provide direct support to junior staff within the service.
