We actively seek to engage people who use our services and our communities in improving the mental wellbeing of the local population. We work closely with other NHS and voluntary sector organisations who provide services and support people who use services and carers.
This role is working within i-access, a successful, countywide drug and alcohol treatment service for Surrey residents delivering tier 2, 3 and 4 substance misuse interventions across 3 main bases and 25 satellite locations. i-access is run collaboratively by SABP, PH / SCC and Via under a cooperative agreement which is one of partnership and transparency and contributes to Surrey Heartlands wider strategic ambitions.

- To actively work with people in implementing the care plan devised by a team professional.
- To act as an advocate and ensure that there is proper regard for dignity, choice, self-esteem, racial, cultural, sexual and religious needs and preferences.
- To participate in assessments, planning, implementation and evaluation of interventions as part of a holistic approach working in a variety of settings.
- To observe and document any changes in wellbeing of the people that they are supporting, reporting to the professional or team manager any new or change in risk or presentation of the person.
- To take appropriate and immediate action to keep people safe in a crisis or safeguarding situation, reporting back to a senior colleague in a timely manner.
- To work within the Positive Behaviour Support framework.
- To work in partnership with people in other health and social care settings.
- To establish and maintain effective relationships with colleagues, other agencies, people who use services and their families.
Assist individuals with eating and drinking. - Support personal hygiene needs and all aspects of personal care, such as bathing/showering and support with continence needs.
- Support physical care needs which may be due to a range of acute or chronic difficulties, and includes manual handling using equipment provided.
- Involve and engage individuals in all aspects of their care.
- Ensure that the dignity and respect of individuals are maintained at all times.
- Act as Shift Leader and delegate responsibilities to colleagues at handover in line with needs and preferences, ensuring the shift planner is updated.
- Support people with their medication.
Contribute to, and record, health checks, risk assessments and care plans, using physical records and/or digital tools (depending on registered home). - Promote and respond to the choices and preferences of the people we support, as detailed in person-centred plans.
- Support an active and enjoyable lifestyle of individuals, by encouraging a range of leisure activities, social activities and holidays, as identified in their person-centred plans.

social networks

Experience

- A driving licence suitable driving in the U.K, and meets the requirements set out by the Trust insurers to drive the Trust vehicle.
