Job overview
We are looking for a Band 7 Advanced Nurse Practitioner to join the team at Deeside Community Hospital. We have two wards - one Care of the Elderly (28 beds) and one Stroke Rehabilitation (18 beds).
The post holder will provide clinical leadership within busy community Hospital where patients are referred from GP or hospital setting. The post holder will work closely with the multi-disciplinary team and other community services and be able to practice at an advanced level of professional autonomy as well as being a good team player.
The role of the Advanced Nurse Practitioner is key to driving standards for professionals and ensuring the delivery of care to patients is exemplary and timely within a proactive community hospital.
Main duties of the job
The postholder will provide nursing expertise, knowledge, and skills in clinical practice that are easily accessible, flexible and provide high quality service to enhance patient care. To include taking the lead role in the advance clinical assessment of inpatients who have highly complex and/or chronic presentations.
Determine clinical diagnosis and treatments as indicated and maintain records as an autonomous practitioner liaising closely with medical practitioners, making best use of clinical skills, within the hospital and enabling the service to respond flexibly. It is anticipated that 80% of time spent will be within the clinical area.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
