We are looking for an experienced Advanced Clinical Pharmacist to join our Scarborough, Whitby and Ryedale (SWR) services in our North Yorkshire locality. The post holder will work with the SWR community teams and support the wards at cross lane hospital in Scarborough.
This is an exciting time in Tees, Esk and Wear Valleys NHS Foundation Trust (TEWV) as we expand our Medicines Optimisation services into our community teams and primary care, in line with the ambitions of the NHS 10 Year Plan. The new plan sets out a vision for moving care closer to home, embracing digital innovation, and prioritising prevention and early intervention, making this a pivotal role in shaping the future of community mental health care.
You must have the Right to Work in the UK in order to be successfully appointed to this role. Please note, this role does not meet the required eligibility criteria for sponsorship under the Skilled Worker visa and therefore the Trust is unable to offer sponsorship for this particular role.

If you are a pharmacist experienced in or interested in mental health, this could be the post for you as we transform our medicines-related services for community patients. This is a very patient-focused role, with an emphasis on:
- Improving processes to reduce risks with medicines and enhance the patient experience, especially at interfaces of care.
- Ongoing monitoring and supply of medication.
- Leading and supporting key workstreams, including discharge follow-up, safe transfer of prescribing, and service user education.
- Providing expert advice on complex medicines management queries, supporting colleagues in primary care, community teams, and voluntary sector partners.
- Delivering patient-centred consultations, education sessions, and workshops, empowering service users and carers to understand and manage their medication.
- Contributing to the development and implementation of innovative, integrated care pathways, working closely with Primary Care Networks and local stakeholders.
The TEWV community mental health pharmacy team in North Yorkshire currently consists of two clinical pharmacists and five pharmacy technicians. The team works with community mental health teams, and primary care delivering medicines optimisation expertise. You will be supported by a lead pharmacist and lead pharmacy technician and be part of the wider North Yorkshire Pharmacy Team.

As an Advanced Clinical Pharmacist in our community mental health teams, you will:
- Be at the forefront of the shift to neighbourhood-based, integrated care, working closely with GPs, mental health professionals, and voluntary sector partners.
- Play a key role in prevention, supporting people to manage their mental and physical health, reduce risks associated with medicines, and improve outcomes for those with complex needs.
- Support the plan’s focus on tackling health inequalities, ensuring that services are accessible and effective for all, especially those in underserved or deprived areas.
- Work on the Cross Lane Hospital wards when required.
We are committed to developing our staff and there will be an opportunity to undertake the Post Graduate Certificate in Psychiatric Therapeutics, Non-Medical Prescribing certificate (if you are not already a prescriber); as well as developing your leadership knowledge and skills.

Due to the nature of the role, it is essential that applicants are able to travel independently across the North Yorkshire locality and have access to a suitable vehicle for business purposes. If necessary, adjustments can be considered in accordance with the Equality Act 2010. A lease car scheme is in operation.
The ability to work flexibly and manage your own workload is essential. You will be assigned a team base in the vicinity of the community team(s) you are working with.

- Masters degree in Pharmacy or equivalent
- Member of the General Pharmaceutical Council (GPhC)
- Postgraduate qualification in clinical pharmacy
- Post graduate qualification in psychiatric pharmacy or willing to work towards
- Evidence of Continuous Professional Development
- Postgraduate Pharmacist independent/supplementary prescribing practice certificate

- Attended Leadership and Management training or have (or studying for) formal Leadership qualification (e.g. NHS LA, CPPE)
- Associate or Full Credentialed Member of College of Mental Health Pharmacists
- Completion of CPPE Mental Health module
