Are you a Learning Disability Nurse looking to help make a difference to your community?

The Sirona Adult Learning Disability Health Service is a centre of excellence for specialist learning disability health needs. We are organised in 3 localities where multidisciplinary teams enable our service users to access health services through direct care, liaison with other services and offering support and training on how to work with people who have a learning disability and how to make reasonable adjustments.

We  support adults with Learning disabilities (LD) who present with complex physical health needs, this may also include an element of behaviours that challenge and/or Mental Health needs.

You will be supported within the multidisciplinary team (MDT), working towards a person centred approach to formulate, implement and evaluate appropriate care plans to improve positive health outcomes.

Service hours: Monday to Friday 9am to 5pm
To work across Bristol, South Gloucestershire and North Somerset depending on Service need

Main duties of the job

  • To maintain an active caseload of service users, working jointly with mainstream services, to promote better access to health care and improved outcomes.

  • To give nursing support and advice in relation to health care to people with an LD and their families/carers.

  • To assess, develop, implement and evaluate of MDT interventions.

  • To undertake, record and review risk assessments

  • To work autonomously, prioritise and organise workload

  • To utilise and participate in regular clinical and peer supervision using reflective practice in accordance with clinical governance.

  • To develop and maintain positive therapeutic relationship with service users and carers.

Essential criteria
  • RNLD qualification
  • To be able to drive and have access to a car
Desirable criteria
  • Post-graduate qualification in a relevant area

Skills, Experience & Knowledge

Essential criteria
  • Wide range of communication skills with clients and carers, colleagues, other statutory, private and voluntary agencies
  • Experience of working with people with LD and their carers’
  • Experience relevant to community working, working as part of a multi-disciplinary team and with other agencies.
  • Ability to work independently and autonomously, prioritising own workload
  • Experience of completing holistic assessment to meet health needs for people with a learning disability
  • Comprehensive IT skills
Desirable criteria
  • A minimum of two years post qualification experience working with people with a learning disability managing complex health needs.
  • Experience of line managing or providing supervision to others
