Job overview

 As a Specialist Nurse your role is to provide care, advice and education to patients, families and fellow practitioners within a specialist area of nursing.

This is a broad and multifaceted role requiring a blend of clinical expertise, management skills and leadership abilities to deliver high quality nursing care. In collaboration with the wider healthcare team, you will help to formulate treatment plans and evaluate patient progress regularly.

Sharing your findings and recommendations with the team is part of your duty and this ensures patient constantly receive the best possible care.

  • Work collaboratively to influence and drive the development of the service.
  • Provide high quality person centered care which always considers people’s safety, privacy and dignity using your clinical knowledge that is evidence based.
  • Undertake assessment and management of service users to incorporate pharmacological considerations, where appropriate making clinical decisions regarding service user management and clinical outcomes

Main duties of the job

  • Provide high quality clinical practice. Be proactive in clinical decision making, underpinned by theoretical and practical knowledge and be able to demonstrate improved patient care outcome
  • Be aware of, and act in accordance with the NMC Code of Conduct.
  • Undertake holistic assessment of patients and carers needs. This will involve planning, implementing and evaluating the care delivery according to changing healthcare.
  • Utilise clinical skills in assessment and or formulating a plan of care
  • Work within own scope of practice and in collaboration with the multi-professional team to instigate, evaluate and modify a range of diagnostic testing /interventions (service specific).
  • Demonstrate sound knowledge in relation to pattern of disease or disorder (service specific), markers of condition progression and range of treatment available at each stage of disorder or condition.
  • To work collaboratively utilising history taking and examination skills within multidisciplinary team, guidelines and protocols.
  • Be able to assess and re-evaluate the clinical situation as the patient’s condition changes.
  • Anticipate likely potential physical and psychological problems caused either by the condition or by treatment

Detailed job description and main responsibilities

  • Demonstrate and further develop specialist knowledge in Glomerulonephritis (GN), Vasculitis, and Polycystic Kidney Disease, including the interpretation and understanding of relevant blood test results.
  • Work collaboratively with the multidisciplinary Renal team, including Nephrologists, to enhance specialist knowledge, skills and patient outcomes
  • Support patients in the management of kidney conditions with the aim of relieving and controlling symptoms and reducing disease progression.
  • Develop and apply advanced assessment skills to identify patient needs, providing tailored education, information, advice and counselling.
  • Provide cross-cover for the Remote Monitoring Service as required
  • Liaise with other Renal services to benchmark practice and share learning to promote continuous improvement.
  • Support service delivery across the Berkshire Kidney Unit, including attendance at clinic across East and West Berkshire – Royal Berkshire Hospital, West Berkshire Hospital, Bracknell Health Space, Townlands Hospital and Windsor Dialysis Unit.
  • Provide expert nursing care and specialist advice for patients diagnosed with Vasculitis, Glomerulonephritis (GN) and Autosomal Polycystic Kidney Disease

Education, Qualifications & Training

Essential criteria
  • Registered Healthcare Professional (NMC)
  • First Level degree (HEI Level 6) or equivalent, in undergraduate base profession
Desirable criteria
  • Specialist post-graduate education
  • Qualifications, or training relevant to speciality area / clinical practice
  • Mentoring, Facilitating Workplace Learning or Clinical Supervision Qualification
  • Leadership course / qualification
  • Registered with NMC as an Independent / Supplementary Prescriber (if required within the service) or willing to undertake training

Experience

Essential criteria
  • Demonstrable experience of managing a varying aspects of a patient’s care within a Renal Speciality
  • Experience of supervising, teaching, mentoring and developing staff
  • Experience of working in partnership with all levels of staff, evidencing the ability to plan and co-ordinate the provision of care
  • Able to demonstrate ability to deliver training and education to patients
Desirable criteria
  • Experience of developing and implementing policies and protocols
  • Demonstrate an awareness of risk management

Skills, Knowledge & Abilities

Essential criteria
  • Demonstrable evidence clinical skills required within service / speciality
  • Ability to analyse and interpret information from patient assessment and interventions, and freedom to act in managing patients ‘care
  • Clinical reasoning, problem-solving and decision-making skills
  • Demonstrable ability to work in a busy environment and resilience when the service is under pressure.
  • To be competent to deal calmly and professionally with patients, relatives and work colleagues in a confidential and sensitive manner
  • Ability to work unsupervised and on own initiative, as well as part of a multi-disciplinary team
  • Understanding of the theory and application of Clinical Governance
  • Commitment to CPD practices for own development, and the development & support of others
  • Demonstrates attitudes and behaviours which reflect the organisational CARE values and role model these to others.
