Job overview

The post holder is a Senior Physiotherapist (Band 6) with the clinical skills, expertise, and knowledge to support Community Reablement, the Community Physiotherapy Service, and the Falls Service. 

Intermediate Care is a multidisciplinary service that helps people remain as independent as possible. It provides support and rehabilitation to individuals at risk of hospital admission or those who have recently been discharged. The aim is to ensure timely transfer from hospital to the community while preventing avoidable admissions to hospital or residential care. 

Wokingham Intermediate Care is a team of physiotherapists, occupational therapists, and senior therapy assistants who treat people in their own homes, supporting them to regain the highest possible level of independence. This supportive multidisciplinary team works with patients across Wokingham in close collaboration with social services, voluntary agencies, and GPs. 

This post involves working six days within a shift pattern as part of the Intermediate Care service

Main duties of the job

  • Complete the timely assessment and management of referrals with multiplco-morbidities and complex rehabilitation needs.   
  • To support triage the referrals for intermediate care services   
  • Provision of equipment to promote independence and safety.  
  •  Hold a caseload and work autonomously  
  • Work flexibly to ensure urgent referrals are adequately managed 
  •  Supporting colleagues in the delivery of compassionate, safe, and structured care 
  •  Provide guidance and appropriate delegation within the team 
 
  • To support the supervision of more junior members of staff and provision of education and clinical physiotherapy advice to staff /students in Intermediate Care.

Essential criteria
  • Diploma/Degree in Physiotherapy
  • State Registration
  • CSP membership

Continuous Professional Development

Essential criteria
  • Evidence of CPD in a portfolio including attendance at recent post-graduate courses relevant to the clinical field / reflective practice in relevant clinical field

Previous Experience

Essential criteria
  • Experience of lone working
  • Experience of working in an intermediate care setting.
  • Experience of working in a multi disciplinary team setting.
  • Experience of liaising with Social Care in the UK
  • Experience of providing a broad range of interventions for a complex caseload
  • Use of goal setting and outcome measures
  • Up to date evidence based on clinical knowledge
  • Experience of working with acute and chronic conditions.
  • Evidence of training in rehabilitation of older people
  • Training in the equipment needs of older people
  • Understanding of the models used in Physiotherapy.
