Are you looking for the opportunity to work within an award winning specialist community Falls & Rehabilitation service? We are looking for a skilled and enthusiastic physiotherapist to join our multidisciplinary team providing assessment and rehabilitation for adults with a range of conditions impacting their functional ability. Our friendly and supportive team is comprised of physiotherapists, occupational therapists, specialist nurses & therapy assistant practitioners, who are all passionate about patient centred care and supporting individuals to regain, retain & improvement their functional independence. Having won a NICE Shared Learning Award & finishing runner up in the CAHPO awards 2019 for the developments to our Falls service, the team are listed as an example of good practice: Dudley Falls Prevention Service | NICE This demonstrates our commitment to evidence-based, multifactorial assessment & interventions for our diverse and often complex caseload. As a service we have built upon this success, embedding best practice & seeing the rate of falls related admissions remain below the national & local average. The service works closely alongside the local Rapid Response Team- Own Bed Instead, and Intermediate Care Team colleagues, with opportunities for collaborative working to optimise patients’ rehabilitation journeys. The role will work delivering tailored, specialist treatment plans both in patients’ homes and from our specialist Rehab Centre at Corbett Hospital. 

Main duties of the job

To work as part of the Dudley Rehabilitation Service, in the Falls and Community Rehabilitation pathway, providing effective and efficient physiotherapy to patients with a variety of physical conditions as part of an MDT. Duties will include assessment, therapy (both group and/or individual as appropriate), developing tailored therapy programmes through to discharge, as well as appropriate delegation of work to assistants. Working autonomously as a lone practitioner within people’s own home, using evidence based/client centred principals to assess, plan, treat and evaluate interventions.

Qualifications & Education

Essential criteria
  • Diploma / degree in Physiotherapy
  • Registered with HCPC
  • Evidence of relevant post-graduate experience
Desirable criteria
  • Member of CSP

Knowledge and Training

Essential criteria
  • IT Training / knowledge
  • Good knowledge of falls prevention and falls risk factors/complex comorbidities
  • Awareness of relevant national and local guidelines
  • Commitment to personal and team development
  • Commitment to undertake appropriate training
  • Willingness to work towards competencies in vestibular assessment & treatment

Experience

Essential criteria
  • Completed junior rotations in core areas or equivalent rehabilitation experience across a wide variety of clinical settings
  • Experience of working with physically impaired patients and fallers
  • Experience of rehabilitation work within an acute/community rehabilitation setting
  • Relevant experience at post graduate level
  • Experience of working in a Multidisciplinary team
Desirable criteria
  • Experience at band 6 level
  • Experience of community working
