GP - Safe@Home Service

NHS Medical & Dental: Salaried GP

CARING FOR PEOPLE - KEEPING PEOPLE WELL

Job overview

We are delighted to share this exciting opportunity for a suitably experienced GP to join the Safe@Home Service.

Safe@home is a new multi-agency and multi-professional integrated urgent response service which addresses a current gap in our range of intermediate care services. Its aim is to provide an immediate and safe alternative to ambulance conveyance, EU attendance and admission to hospital when it is safe and appropriate to do so.

The new service is being developed for Cardiff and Vale drawing upon learning from established services across Wales and England.  The following diagram represents the full service model (The Ambition) we aim to introduce and test as a new multi-agency, multi-professional approach to provide early senior clinical intervention and support in the community.

Main duties of the job

This is an exciting prospect for a GP to contribute and further shape the implementation of a new service model, aligned to the UHBs 6 Goals Programme for Urgent & Emergency Care, whilst working alongside existing Primary and Community Care services.

In your role,  you will provide clinical oversight and supervision for all staff working within the Service to support patients to remain at home with the right treatment and care, enabling them to recover from the medical crisis and avoid harm associated with an inappropriate admission and long length of stay.

Given the developmental nature of this opportunity, the post holder may be required to undertake duties which may vary to those outlined in the job description.

If you would like to discuss this opportunity further please get in touch.
