Job overview

We are excited to offer an opportunity for two experienced and motivated rotational Pharmacists to join our dynamic Pharmacy team at Glangwili General Hospital.
 
You will be working across a variety of general medical and surgical wards, within the Emergency Department and the Women and Children's wards, delivering a proactive, patient-centred clinical pharmacy service. You will be key members of the multidisciplinary team, contributing to safe, effective and evidence-based use of medicines to improve patient outcomes.  
 
You will deliver a high quality, patient focused clinical pharmacy service by carrying out comprehensive medicines reconciliation, provide expert advice on the safe, effective and appropriate use of medicines, assess prescriptions for clinical appropriateness and support prescribers in complex decision-making, supply medication in a timely manner.  
 
You will support the Lead Pharmacist by contributing to the development and implementation of guidelines and protocols, participate in clinical audit, incident reviews and quality improvement projects to enhance patient care.

Main duties of the job

As part of a dynamic and forward-thinking pharmacy team you will:

  • Provide expert clinical pharmacy support to CDU and ED
  • Counsel patients to optimise their understanding and adherence to treatment. 
  • Work closely with multi-disciplinary teams to ensure safe and effective medicines us
  • Actively participate in board and ward rounds to promote safe prescribing practices and facilitate discharge planning
  • Support antimicrobial stewardship and medicines optimisation initiatives
  • Contribute to service development and quality improvement projects
  • Provide education and training  by supervising and mentoring junior pharmacists, pharmacy technicians, pharmacy students as well as clinical ward staff

Hywel Dda University Health Board plans and provides NHS healthcare services for people living in Carmarthenshire, Ceredigion, Pembrokeshire, and bordering counties.

We have over 13,000 staff and together we provide primary, community, in-hospital, mental health and learning disabilities services.

We work in partnership with the three local authorities, as well as public, private and third sector colleagues, including our valued team of volunteers.

  • Four main hospitals: Bronglais Hospital in Aberystwyth; Glangwili Hospital in Carmarthen; Prince Philip Hospital in Llanelli; and Withybush Hospital in Haverfordwest
  • Five community hospitals: Amman Valley and Llandovery hospitals in Carmarthenshire; Tregaron Hospital in Ceredigion; and Tenby and South Pembrokeshire hospitals in Pembrokeshire
  • Two integrated care centres: Aberaeron and Cardigan in Ceredigion, and several other community settings
  • 47 general practices (six of which are health board managed practices); dental practices (including four orthodontic); 97 community pharmacies; 43 general ophthalmic practices; and 8 ophthalmic domiciliary providers
  • Numerous mental health and learning disabilities services

Qualifications and Knowledge

Essential criteria
  • Masters degree in pharmacy (4 years) or equivalent experience or qualifications
  • Registered with the GPhC
  • Professional knowledge of medicines management and process in an integrated NHS
  • Experience of teaching & supervision of pharmacists, technicians, assistants and students
  • Diploma in Clinical Pharmacy or equivalent experience or qualifications
  • Knowledge of specialist area as described in job advert.
Desirable criteria
  • Accredited pre-registration Tutor
  • Accredited Diploma Pharmacy Tutor
  • Independent Presriber

Experience

Essential criteria
  • Highly developed knowledge and experience of healthcare
  • Demonstrate achievement in at least one area of specialist medicines management
Desirable criteria
  • Demonstrate achievement in more than one area of medicines management

Laguage skills

  • Welsh Speaker (Level 1)
