This is a keyworker and pathway leadership role, supporting prostate cancer patients from diagnosis through treatment, surveillance and survivorship. The post holder will work across the entire prostate pathway, ensuring timely diagnostics, effective MDT functioning, high‑quality patient communication, and protocol‑driven follow‑up in line with national cancer standards.
This role is being established to convert recent diagnostic capacity improvements into sustained pathway performance, improved patient experience, and reduced harm from delays.
Interviews to be held on 16/7/2026
Main duties of the job
The CNS will:
- Act as the designated keyworker for prostate cancer patients, providing consistent, personalised support across diagnosis, treatment and follow‑up
- Lead and coordinate the prostate cancer diagnostic pathway, including straight‑to‑MRI and LATP biopsy pathways
- Play a central role in MDT preparation, attendance and follow‑up, ensuring complete, high‑quality clinical information and timely communication of outcomes
- Deliver CNS‑led clinics, including PSA surveillance, active monitoring and treatment follow‑up, supporting the Health Board’s wider follow‑up transformation agenda
- Support patients undergoing surgery, radiotherapy and systemic therapies, providing education, symptom management and early escalation of concerns
- Deliver holistic needs assessments, treatment summaries and survivorship care planning in line with national cancer standards
- Support the rollout of remote monitoring and digital tools (e.g. Patients Know Best) for prostate cancer follow‑up
- Work collaboratively across Urology, Radiology, Pathology, Oncology, Cancer Services and Primary Care to improve pathway flow and experience
- Contribute to service development, audit, quality improvement and performance recovery, with a clear focus on Faster Diagnosis Standard and 62‑day USC compliance
