Job overview

An opportunity has arisen for a permanent Lead Enhanced Clinical Practitioner to join the community team to support Carmarthenshire's Clinical Streaming Hub based in Intermediate Care at Eastgate in Llanelli.  

This post will require an enthusiastic, motivated and dynamic practitioner that has a broad understanding and experience of acute and chronic conditions. Experience of autonomous practice in the emergency/urgent/Community arena is essential.

The team has the single goal of ensuring the best outcomes for our patients. We actively encourage continuing professional development, leading to the enhancement of your clinical skills and theoretical knowledge. You will have the opportunity to develop and consolidate skills in advanced clinical assessment and treatment and be involved in improving integration across the community.

If the successful candidate does not meet all the criteria for this Band 7  Enhanced Clinical Practitioner post, an  Annex 21 can potentially be applied where the candidate will work towards the necessary  competence level required to fulfil the post.

Main duties of the job

The service is constantly developing, in line with changes in local and national healthcare provision and as such we are looking for a forward- thinking, competent, and caring Practitioner with the skills and experience needed for triaging and assessing patients and carrying out treatments working closely with the Advanced Clinical Practitioners and the Intermediate Care MDT.

The post holder will be expected to undertake in depth advanced assessments and have the experience and training to develop care plans with the community/primary and acute MDT. Diagnostic skills and the ability to multi-task are essential.

The successful candidate will be committed to service improvements with an acceptance of change, and desire to work in a positive environment that embraces the challenges urgent care faces as the service moves forward.

You will demonstrate a calm and confident approach to your work, demonstrating excellent clinical and communication skills and the ability to manage and prioritise workloads effectively. Team working and a flexible approach to work is important to ensure service provision can be delivered efficiently and effectively.

Currently, the role involves shift work covering 7 days a week, day shifts between  08:00-20:00.

Working for our organisation

Hywel Dda University Health Board plans and provides NHS healthcare services for people living in Carmarthenshire, Ceredigion, Pembrokeshire, and bordering counties.

We have over 13,000 staff and together we provide primary, community, in-hospital, mental health and learning disabilities services.

We work in partnership with the three local authorities, as well as public, private and third sector colleagues, including our valued team of volunteers.

Our services are provided in:

  • Four main hospitals: Bronglais Hospital in Aberystwyth; Glangwili Hospital in Carmarthen; Prince Philip Hospital in Llanelli; and Withybush Hospital in Haverfordwest
  • Five community hospitals: Amman Valley and Llandovery hospitals in Carmarthenshire; Tregaron Hospital in Ceredigion; and Tenby and South Pembrokeshire hospitals in Pembrokeshire
  • Two integrated care centres: Aberaeron and Cardigan in Ceredigion, and several other community settings
  • 47 general practices (six of which are health board managed practices); dental practices (including four orthodontic); 97
  • community pharmacies; 43 general ophthalmic practices; and 8 ophthalmic domiciliary providers
  • Numerous mental health and learning disabilities services

Detailed job description and main responsibilities

This post is advertised as a  Band 7, however,  applicants who do not currently hold all the necessary competencies to a Band 7 level are encouraged to apply and,  if successful, will be appointed under Annex 21 rules whereby 75% of the top of Band 7 will be payable in the first year.  Applicants would be supported to develop their competencies required to fulfil the role within a 12 month period. Following successful completion and successful attainment of the skills and competencies for the role, the post holder will then be paid at Band 7  (minimum point on the scale). Failure to achieve the competencies in the designated timeframe would follow Workforce policy.

As part of the Annex 21 process, there is no financial penalty to those who apply for this role where the percentage will cause a reduction in pay, subsequently a local protection would be put in place until the pay has caught up.

Qualifications and Knowledge

Essential criteria
  • Registered practitioner (e.g NMC/HPC)
  • CPD to Masters level
  • Relevant Level 7 Module covering enhanced assessment, clinical examination and decision making skills
  • ALS /EPALS and in-house Trauma Training or ATLS (If relevant to Service)
  • Level 7 module or equivalent covering assessment and autonomous management of minor injury and illness presentations (If relevant to service)
  • Evidence of developing and maintaining a professional portfolio mapped against the four pillars of enhanced clinical practice, demonstrating competence, reflective practice, and ongoing professional development
  • Specialist knowledge within own scope of practice of enhanced clinical practice and supports the development of colleagues through clinical supervision, education and shared learning
  • Knowledge of applying Clinical Governance principles to support safe, effective and evidenced-based practice
  • Actively contributes to clinical governance activities, including audit, incident learning, quality improvement and implementation of best practice
  • Maintains an up-to-date understanding of national and local policy, guidance and emerging issues relating to enhanced clinical practice in Urgent and Emergency Care
  • Administration of agreed drugs in accordance within locally agreed patient group directions (PGD’s)

Experience

Essential criteria
  • A&E Nursing / Urgent Care / Community or Primary Care experience - working in a senior capacity as part of the MDT
  • Evidence of managing clinical workload
  • Experience of providing specialist clinical advice within the multidisciplinary team
  • Experience of actively contributing to clinical governance activities, including audit, incident learning, quality improvement and implementation of best practice.
  • Experience of critically applying clinical knowledge, evidence and professional judgement to support safe, effective and person-centred decision-making
  • Evidence of leading or contributing to quality improvement methodology resulting in service improvement
  • Experience of leading or significantly contributing to service redesign, pathway development or implementation of service improvement
  • Experience of delivering education programmes
  • Digital healthcare or virtual care experience
  • Experience of providing clinical supervision and supporting the development of juniors
  • Experience of coordinating patients flow and clinical decision-making across multidisciplinary teams
  • Experience of supervisory management
Desirable criteria
  • Development of expertise within urgent care areas.
  • Relevant experience in the field of palliative care/frailty/Acute Medicine/ED
  • Evidence of working as an autonomous practitioner
