Job overview

An opportunity has arisen for a suitably qualified enthusiastic and motivated nurse to join our friendly and supportive Community Team for People with a Learning Disability (CTLD) in Hywel Dda University Health Board, based in Llanelli.

 

Main duties of the job

You will be expected to work proactively and creatively, using your skills and experience as part of a multidisciplinary team to deliver high quality, evidence based nursing care to adults and young people with a learning disability in partnership with them and their families/carers. You will have the opportunity to work closely with other colleagues in the multi-disciplinary team, and will provide specialist advice, support and training to colleagues, families and carers in addition to the people on your caseload.  

 You will also be supported to develop your own clinical competency profile and to develop skills in specialist areas of practice.   We offer regular supervision, professional development and access to peer Nursing support.   

We would expect any interested candidates to be up to date with current learning disability policy/legislation and demonstrate an ability to improve service delivery by promoting change in health related projects.  We are keen to support nurses who are willing to share new ideas and suggest improvements. We have close links with Improvement Cymru and have access to dedicated in-house project management support.

Hywel Dda University Health Board is the planner and provider of NHS healthcare services for people in Carmarthenshire, Ceredigion, Pembrokeshire and its bordering counties. Our 11,000 members of staff provide primary, community, in-hospital, mental health and learning disabilities services for around 384,000 people across a quarter of the landmass of Wales. We do this in partnership with our three local authorities and public, private and third sector colleagues, including our volunteers, through: 

Four main hospitals: Bronglais General in Aberystwyth, Glangwili General in Carmarthen, Prince Philip in Llanelli and Withybush General in Haverfordwest;

Seven community hospitals: Amman Valley and Llandovery in Carmarthenshire; Tregaron, Aberaeron and Cardigan in Ceredigion; and Tenby and South Pembrokeshire Hospital Health and Social Care Resource Centre in Pembrokeshire;

48 general practices (six of which are managed practices), 47 dental practices (including three orthodontic), 99 community pharmacies, 44 general ophthalmic practices (43 providing Eye Health Examination Wales and 34 low vision services) and 17 domiciliary only providers and 11 health centres;

Numerous locations providing mental health and learning disabilities services;

Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.

Person specification

Qualifications and Knowledge

Essential criteria
  • Registered Nurse (RNLD, RGN, or RMN) with a current NMC registration
  • Good understanding of current policy and practice in relation to people with Learning Disabilities
Desirable criteria
  • Registered Nurse Learning Disabilities (RNLD)
  • Teaching Qualification or equivalent, or recent experience in the provision of training to health staff

Experience

Essential criteria
  • Experience of working with adults with a learning disability
Desirable criteria
  • Experience working with community LD teams and Primary care services

Language Skills
