Job overview
An exciting opportunity has arisen at Mid Cheshire hospital for an enthusiastic, motivated and dynamic individual to join the Colorectal Cancer Nurse Specialist Team as a Colorectal Cancer Clinical Support Worker.
The Cancer Clinical Support Worker will form an integral part of the multidisciplinary team approach in caring for the physical, social and psychological needs of patients diagnosed with cancer and their carers throughout the care treatment pathway and after care support.
You will work in close unison with the Clinical Nurse Specialists (CNSs) to ensure the coordination and management of patients at all points of their treatment including stratified follow-up and will be able to signpost patients to support their self-management. You will be responsible for the management of IT systems that support the cancer pathway and be a key contact for patients.
The post holder will work independently within the remit of the role but under the guidance and supervision of the Clinical Nurse Specialists.
Main duties of the job
· To Support the Colorectal Nursing Team in the delivery of nursing care and services to patients from pre-diagnosis to end of life care in primary & secondary care at any point of their colorectal cancer journey;
· Provide support to the Colorectal Nursing Team, contributing to Multi-disciplinary Team (MDT) discussion, patient assessment and reviewing care plans established by Colorectal Nurses and wider MDT;
· Providing support with clinical administration, including collating expediting results and supporting the Cancer Pathway Co-ordinator with the organisation of MDT discussion lists;
· Supporting the Colorectal Nursing Team with activities undertaken to raise awareness of the signs and symptoms of colorectal cancer;
· Supporting the Personalised Care, Health and Wellbeing Workshop;
· Assisting with the day to day running of the service including providing information for patient and carers and where appropriate sign posting to most relevant service.

Qualifications
Essential criteria
- GCSE English and Maths
- Level 3 Health and social care or equivlent
- Previous experience in cancer care
Experience
