Job overview
The diabetes team is based in the acute hospital trust and consists of a Consultant Diabetes nurse, an advance nurse practitioner, Diabetes specialist nurses working in outpatient clinics and community . We work closely as a multi-disciplinary team including our administration team and the consultant diabetologists providing a Monday to Friday service. We are developing our well established insulin pump service over both hospital sites and are seeking someone with pump experience or willing to develop their knowledge in this area of diabetes management.
Main duties of the job
Provide leadership in the field of diabetes and draw on advanced knowledge and exercise professional skills to develop and deliver high quality service to people with diabetes.
· Act as an expert practitioner, overseeing, delegating and supporting care carried out by other members of the diabetes team where required
· Assist and facilitate the development of systematic review and appropriate management of all people with diabetes
· Work in collaboration with other professionals and users by offering highly specialist advice to develop long term strategy plans
· Responsible for managing the staff rota for the group diabetes education sessions.
· Work with other members of the diabetes nursing team.
· Work collaboratively and co-operatively with clinical colleagues to develop an innovative high quality service, which enables individualised care to be facilitated and delivered to people with diabetes and their carers/families.
· Assist members of the diabetes nursing team in developing knowledge and ideas to promote innovative practice by influencing good evidence based practice.
· Responsible for identifying the appropriate level of work and delegating to junior nursing staff working within the diabetes team
