To facilitate patient referral pathways across the ENT & Audiology Service in all sub-specialties for Audiology. To monitor referral and triage pathways for Audiology.  They will also be required to monitor PTL’s, data quality, diagnostic test actioning, flagging capacity issues, any alerts within UHP Trust protocol timeframes and monitor diagnostic test review turn-around across Audiology.

PRIMARY DUTIES & AREAS OF RESPONSIBILITY

1.Continued review and monitoring of various referral pathways within  Service Line.

1.     Work with a range of clinical and non-clinical staff within the ENT & Audiology Service Line. 

2.     Provide and receive complex information about patient care and interpret instruction to relay to varying recipients specific to clinical referrals or clinical results.

3.     The post holder is expected to liaise with Audiologists to ensure appropriate navigation through specific referral pathways.

4.     To have a good knowledge and understanding of DRSS and UHPNT referral processes, and referral targets matching ie diagnostic standard/DM01 etc specific to Audiology.

5.     To understand basic medical terminology relating to ENT & Audiology to enable timely and accurate relaying of information,

6.     To work closely with other team members when chasing outstanding actions and processes, ensuring strong and clear communication throughout.

7.     Following up and tracking outcomes of requests and acting on these in a timely, appropriate manner.

8.     Provide basic advice and refer on or sign-post to other sources of support.

9.     Co-ordinate and organise reviews as required and to exchange that information clearly and concisely dealing with anxiety, cultural or language difficulties.

10.  Plan and organise complex programmes for co-ordination of patient pathway.

11.  Facilitates communication between different clinical services to provide timely service provision.

12.  Document and monitor all aspects of care coordination and service delivery, supporting data collection for audit.

13.  To ensure that high standards of documentation and care planning for patients are maintained.

 

2. Continued review and oversight of Imaging reports requiring action within Service Line.

14.  The post holder is expected to liaise with the Audiologists to ensure appropriate timely review of diagnostic test reports.

15.  Provide and receive complex information about patient care and interpret instruction to relay to varying recipients specific to clinical referrals or clinical results.

16.  To have a good knowledge and understanding of  booking and reporting software and processes.

17.  To work closely with other team members when chasing outstanding actions and processes, ensuring strong and clear communication throughout.

18.  Ensure accurate data is rapidly captured and recorded to meet the standards of the management of patients, and to fulfil the relevant information requirements.

19.  Create databases to collect and monitor information, maintain and update.

20.  Facilitate collection and analysis of data by collating information via various electronic systems and face to face contact with members of the relevant Clinical or Service Line management bodies.

21.  To be proficient in the use of Auditbase, CRIS and iPM systems.

 

3. General Responsibilities

22.  Works closely to develop good working relationships with Audiologists, Secretaries, nurses, consultants and the wider ENT & Audiology Service Line.

23.  Develop appropriate systems to manage their own workload.

24.  Operates on their own initiative whilst bearing the overall Service Line aims and responsibilities.

25.  To ensure that a clear understanding is obtained surrounding data quality, data protection, patient information management systems (iPM) and Auditbase and be able to work within these guidelines at all times.

26.  Ordering patient information and updating as necessary.

27.  Assist with teaching and educating other members of staff.

28.  Ensuring that patient experience remains the focal point for service improvement.

29.  Achieving UHPNT specific targets relating to referral triage, Referral to Treatment, DM01 diagnostic targets and governance processes/protocols.

 

4. Personal Qualities

30.  Demonstrate self-directed learning, actively seeking role development opportunities to enhance practice, knowledge and role progression.

31.  Identify personal education needs and skills development with Line Manager.

32.  Understand that there may be exposure to distressing situations when reviewing patient information and be aware of own need to seek appropriate support/advice.

33.  Demonstrate an awareness of the limits of own practice and knowledge and when to seek appropriate support/advice.

34.  Will be able to demonstrate excellent communication skills.

35.  Will need to demonstrate high levels of organisational skills in order to set up new role/systems.

36.  Concentration required for data analysis and tracking patients with frequent interruptions requiring urgent attention and re-prioritisation of work.
