Detailed job description and main responsibilities

To provide specialist nursing expertise, facilitating evidence-based, patient-centred care and assessment for patients with Parkinson's disease.

To co-ordinate and monitor in-patients / out-patients throughout their assessment and treatment pathway.

To work with internal and external MDT and report or escalate patient wellbeing or deterioration in health.

To promote and support patients and carers to maintain quality of life balance.

To be involved in delivery of relevant programmes of education and development for all levels of staff and to act as a resource for patients and health care personnel.

Essential criteria

Significant recent experience managing patients with Parkinson’s Disease and related conditions influencing the continued use of evidence-based practice.
Significant experience in working with patients with complex needs, in a hospital or community-based setting. Understanding of professional issues.

Evidence of ability to practice as a proficient practitioner and undertake independent assessments, and contribute to the prescribing of relevant individualised care plans

Experience of development and teaching other groups of staff
Evidence of an area of special interest
Initiative and motivation to develop services

You will be a Senior Assessor within our multidisciplinary assessment team, delivering:

Remote developmental histories and child observations (via video calls)
Peer MDT diagnostic decision-making, with the support of the senior clinical MDT where required
Tech-enabled outcome report writing
Outcome calls with parents and families (via video call)

As a Senior Assessor, you have the opportunity to focus on delivering direct care. Furthermore, if you have the relevant experience and skills, you can take on an additional responsibility. We limit this to one responsibility to allow for focused time on that topic, rather than having your time stretched too thinly. When taking on an additional responsibility, your case volume is reduced, making time available for this focused time.

For Senior Assessors, additional responsibility options include supporting more complex cases, ensuring quality assurance, providing training, facilitating family engagement, and contributing to pathway development.
