The UCR service provide a general virtual ward service, supported by digital technology and remote monitoringOur multidisciplinary teams ensure that patients receive hospital-level care at home safely and in familiar surroundings, helping speed up their recovery while freeing up hospital beds for patients that need them most. Main duties of the job

The Urgent Community Response (UCR) Service provides clinical interventions (including intravenous therapy) as part of a multidisciplinary team comprising of Medics, Nurses, Paramedics, Physio and Occupational Therapists for admission avoidance pathways (2 hour response and general virtual wards), in line with the national specification for UCR and general virtual wards.   

Operating 365 days per year, the service receives referrals from a range of health and care professionals and triages, assesses, and delivers care to the patient over several days to ensure the patient can remain safely at home.  

 Our multidisciplinary teams work flexibly to provide a 7-day service, 365 days a year, between 8am and 8pm.  

A key part of the role is leading on the co-ordination of patient experience outcome measures for the service and providing daily reports on staffing data for the leadership team.

Creating and maintaining the roster via health roster for the team every 4 weeks. reviewing the roster to ensure the service is a safely staffed according to the service template and budget.

Preparing agendas, booking recurrent meetings across both sites on behalf of the operational lead and wider team, taking formal meeting minutes for a range of clinical profession.

a     Using a range of software packages as appropriate, e.g. Word, Excel and PowerPoint to create trackers, presentations and reports, including gathering data from System 1 for KIPI's and reports.

Qualifications and/or professional registration

Essential criteria
  • General standard of education to GSCE level or equivalent
  • NVQ level 2 or equivalent (health and social care or business administration) or be willing to undertake
Desirable criteria
  • Audio typing

Experience

Essential criteria
  • Recent secretarial or administrative experience
  • Evidence of consistent pattern of learning from education, training and experience
  • Experience of typing and producing documents e.g. letters and reports
