Leg Ulcer Clinic Nurse

Trosolwg o'r swydd
We are looking for a Leg Ulcer Clinic Nurse. Provide has lower limb clinics in Maldon, Braintree and Chelmsford. The successful candidate will be expected to work across the clinics assessing and reviewing patients with lower limb wounds often with complex health needs.
The aim is to assess patient within 2 weeks as per NWCS lower limb recommendations.
Assist in managing the caseload.
Liaise with the tissue viability team.
Liaise with other health care professionals where further input is required to support patients.
Prif ddyletswyddau'r swydd
- To work as an accountable and autonomous practitioner working to pro-actively monitor and anticipate changing needs of patients with lower limb wounds.
· To provide advanced clinical care using high level assessment to make a diagnosis, initiate medical/nursing treatments, refer appropriately and co-ordinate the patients journey across all parts of the health, social care and voluntary sector systems whether in clinic or home setting.
· Improve clinical outcomes for patients by enabling them to function independently by increasing their choice to remain in their own home, community and reduce the need for, or prevent inappropriate hospital admission.
· To assist in providing a clinic leg ulcer service for patients in the Mid Essex locality. This is to include clinical care, education and training for patients and staff.
- To develop, implement and evaluate programs of care, setting of standards of care, including nurse prescribing.
- Maintain a high standard of patient focused care reflecting an inter-disciplinary approach to self-management.
- To manage a defined caseload, including liaison with other agencies and where, appropriate the supervision and teaching of staff and/or students.
- Establish and promote joint working with other services across professional and organisational boundaries.
- Develop good communication links with patients and carers and the tissue viability service.
