Job overview
North London Hospice is a registered charity that has been caring for local people since 1984. We are committed to empowering patients with life limiting illnesses, supporting them to achieve the best quality of life possible. As well as our inpatient unit at Woodside Avenue work within the community palliative care-East and West; our support to patients in their own homes in partnership with our specialist Palliative Care teams, providing emotional and practical support to patients, their families and carers.
Social care work within community palliative care team provides psychosocial care to patients, families, and their carers pre & post bereavement. They act as advocates for people with advanced life limiting illness and their family / carers, aiming to enable individuals to make informed choices to manage their health, wellbeing, and lives independently for as long as possible.
Main duties of the job
To work closely with social workers and other multidisciplinary colleagues, to assist and support them in their work and provide timely, creative, person-centred support to patients and their families/carers.

Essential criteria
- NVQ in Social Care or equivalent
Desirable criteria
- Degree in Social Care
Experience and Knowledge
Essential criteria
- Experience working in adult social care services
- Experience of assessment and planning intervention
- Experience of working in a multi-disciplinary team
Desirable criteria
- Experience working with volunteers or unpaid carers
- Experience supporting people through issues of loss
- Experience of managing support workers
Personal Qualities and Abilities
Essential criteria
- Strong interpersonal and communication skills
- Strong organisational and time management skills
- Ability to work well autonomously and as part of a multi-disciplinary team
