Job overview

An exciting opportunity has arisen for an enthusiastic and dedicated person to be part of the  Integrated Supported Home Discharge Team (iSHDT). 

The team is multidisciplinary team  consisting of  Physiotherapists, Occupational Therapists, Social Workers, Social Care Assistants, Rehabilitation Assistants and  Rehabilitation Practitioners .

The team provide reablement support to patients in their own homes  following hospital discharge. You will  have support from and work closely with the multidisciplinary team to provide  patient centred reablement and rehabilitation programmes.

Main duties of the job

We are looking for an individual who has:

  • Relevant experience in a health and social care setting and base level of Occupational therapy/Physiotherapy knowledge
  • Knowledge of patient/client conditions relevant to community rehabilitation
  • Strong administrative and communication skills, able to liaise with a wide range of stakeholders.
  • Ability to work under own direction
  • Able to travel to meet the need of the role

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

 

High Cost Area Supplement – London

This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (minimum payment of £4,313 and a maximum payment of £5,436 per annum pro rata).

Qualifications

Essential criteria
  • Relevant NVQ Level 3 or equivalentCompleted
  • Relevant experience in a healthcare setting and base level of therapy knowledge
  • Care Certificate
Desirable criteria
  • Associate Practitioner Course or Trusted Assessor training
  • Evidence of study of human anatomy and physiology OR work based experience that has developed this knowledge

Experience of providing Rehabilitation

Essential criteria
  • Strong experience of working as a band 3 rehabilitation assistant in a community setting
Desirable criteria
  • Minimum of two years at a band 3
  • Awareness of relevant therapy techniques
  • experience of working in adult social care

Knowledge

Essential criteria
  • An awareness of NHS Plan, NSF and clinical governance priorities
  • Knowledge of patient/client conditions relevant to the community setting
  • Knowledge of multi-professional working
  • Understand the need for professional conduct

Adminstrative Skills

Essential criteria
  • Proficient IT user - familiar with Microsoft Office, RiO or similar
  • Communication and interpersonal skills - developed through working within Health / Social Care
Desirable criteria
  • Experience of working independently and to manage workload
  • Experience of working within highly diverse communities
