Evelina London cares for local children in Lambeth and Southwark and provides specialist services across south east England including cardiac, renal and critical care services. We lead a number of specialist service networks aiming to ensure children are treated locally where possible, but have access to specialist expertise when they need it. Our community services include health visiting, school nursing and support for families of children with long-term conditions.
Our adult community services teams deliver care at the heart of the local communities we serve, working in partnership with GPs, local authorities and other healthcare and voluntary sector organisations. Working with our partners in Lambeth and Southwark, we are focusing on new ways of working to improve care for local patients.
In February 2021 the Royal Brompton and Harefield joined Guy’s and St Thomas’ NHS Foundation Trust, bringing together world-leading expertise in the care and research of heart and lung disease. Job overview
We are looking for a forward-thinking and motivated nurse to join the Neighbourhood Nursing teams across Lambeth and Southwark areas.
You must demonstrate excellent communication and team working skills. Through clinical case management you will care for patients with long-term conditions, those at risk, end of life care needs, and those with supported self-care programmes.
There will be training and opportunities in addition to clinical supervisions and appraisals. The post holder must be able to undertake a prescribing course and advanced assessment courses once in post.
Working on a 7-day shift service (hours between 8.00 – 20.00), this post also includes rotational weekend work (maximum two per month).

Main duties of the job
The Neighbourhood Nurse participates in the clinical case management of patients who have a wide variety of complex, high-risk and supported/self-care conditions.
As a team leader, you will ensure the provision of a high standard of nursing care and health promotion.
Duties include:
- Holistic assessment, planning, implementation and evaluation of care. For example, Chronic Disease Management, Tissue viability, Palliative Care, Nurse Prescribing, Extended and Supplementary Prescribing.
- First contact assessment, care and referral.
- Continuing care, rehabilitation and delivering the National Service Frameworks.
- Providing public health, health promotional programmes to improve health and reduce inequalities.
- Maintaining contemporaneous written and computerised records.

Experience
Essential criteria
- Considerable post registration experience.
- Experience of leading and managing a nursing team.
- Experience of working as a member of a team but also able to work autonomously.
- Experience of teaching and assessing students and other staff members.
Desirable criteria
- Experience of working in primary care / community nursing services.
- Experience as a clinical supervisor.
Skills/Knowledge/Abilities
Essential criteria
- Able to co-ordinate and deliver complex care that involves other agencies
- Ability to analyse and critique assessment and other information and make sound clinical decisions.

Education
Essential criteria
- Registered General Nurse Adult.
- Education to 1st degree level or equivalent.
- Evidence of post-registration education and training in care of older people, long term conditions, palliative and end of life care.
- Nurse prescriber or willing to undertake
- Practise assessor/supervisor or old mentorship qualification or SSSAP.
Desirable criteria
- Clinical specific post registration course e.g. wound management, diabetes, leg ulcer care, continence care and assessment
- Specialist Practitioner District Nursing qualification.
