Job overview

This is an Emergency Medicine Consultant post to join the current Consultant body at The Hillingdon Hospitals NHS Foundation Trust (THH) in providing acute and emergency services to a busy local acute hospital with a comprehensive and diverse range of medical and surgical services.

 

The post holder will play a key role in realising our ambition to build a high quality clinical service.  The successful applicant will be a committed clinician with a track record of excellent clinical service delivery, quality improvement, teaching and training.  It is a requirement of the post that every doctor takes part in annual job planning and annual appraisal as part of the revalidation of cycle.

Main duties of the job

The post-holder will report to the Clinical Lead of Emergency Medicine and be responsible to the Divisional Director for Unplanned Care at The Hillingdon Hospital. The job plan will be negotiated annually. It is a 10 PA post–8.5 direct clinical care (DCC), 1.5 supporting professional activities (SPA), the latter includes clinical and educational supervision, audit and personal CPD. In line with the job planning policy, 1.5 SPA will be paid to all consultants’ core SPA activities (including appraisal, CPD).  A further 1 SPA is available for identified activities required by the department/hospital which will be agreed as part of job planning.  Personal CPD must be included as part of annual appraisal which is mandatory and required for the revalidation process.

The post holder will, together with his or her consultant colleagues and other health care professionals in the Directorate, provide an effective and comprehensive service for Emergency department and support continuing service developments.

Detailed job description and main responsibilities

Clinical
•    Delivery of good quality evidence-based care to patients in the Emergency Department.
•    The consultant must be available to support and help junior staff in the daily handling of clinical as well as non-clinical problems presenting to the department. Structured clinical shifts for ‘all areas of the shop floor within the Departments are a key part of the consultant job plan.
•    Clinical supervision of junior doctors within shop floor sessions, including review of patients and direct management of difficult cases, or sick children. Teaching of junior staff, formal and informal, including EM trainees, and nursing staff when appropriate, including scenario based teaching. 
•    Managing National targets (e.g. the 4 hour target, KPIs) actively and using appropriate strategies to avoid patient breaches, and to be aware of the escalation plan, i.e. OPEL scores. 
•    Managing quality and safety in the department (for example, review of results, review of x-rays, review of clinical incidents and complaints/Datix, quality assurance of junior staff).
•    Attendance/leading at multidisciplinary department meetings and clinical governance meetings.
•    Appraisal of junior staff within the department and assessments for work place based assessments for all trainees, irrespective of their speciality. 
•    Provision and maintenance of up-to-date written guidance and protocol in liaison with consultant colleagues from other specialities if necessary.
•    Ensure procedures and policies are being followed in the department, i.e. Infection Control, Trauma calls etc.
•    To undertake Adult and Child Protection duties in collaboration with other department services provided at the hospital. The Consultant in EM will play a lead role in ensuring systems are in place for the care of other at-risk or vulnerable ED patient groups. To provide support and leadership to the Mount Vernon minor injuries unit as required. 

Management  
•    To collaborate with the Lead Clinician and the Senior Nursing Team in the development and implementation of all relevant policies for the running of the department.
•    To participate as requested in committees relating to the Emergency Department, for example (not an exhaustive list) Clinical Governance, Safeguarding Adults and Children, Transfusion, Resuscitation, Medicines Management.
•    To meet on a specified basis and as required with the Clinical Lead / Divisional Director to confirm, review and develop an agreed job plan for the post that addresses the needs of the service and conforms to the terms and conditions of the Consultant Contract.
•    To co-operate with the consultant team in the recruitment and appointment of junior medical staff in consultation with the Department of People and Organisational Development and the EM Management Team. He/she will also be expected when required to involve themselves with the recruitment and appointment of the Registrar/Middle grade/SHO doctors and, when necessary, of nurses in the department. 
•    Ensuring the use of the computer systems and any recent updates within the department by education and direction of junior doctors’ activity.
•    To get involved in Audit and Quality Improvement Programmes running in the department and across the trust
•    To be an active participant in, and advocate of, the Trust Improvement programme (CARES plus).

Teaching
•    To participate in regular teaching programme for SHO and SAS/Registrar teaching
•    To provide one to one mentoring, skills training, and competency- based assessments for all junior doctors. 
•    The successful candidate will provide Clinical Supervision for approximately five of the locally employed junior doctors within the Department.
•    To support undergraduate teaching in the Trust for medical students from Imperial and Brunel
•    To participate in teaching and supervision of junior doctors in training (FY, ACCS, GPVTS and Specialty training doctors).
