Heart Failure Specialist Nurse

Detailed job description and main responsibilities

The post holder will be based at: QMC & City Campuses.

The post holder will have a Trust-wide responsibility to promote clinical excellence in the care of patients presenting with heart failure.

This post will support the continued implementation of the NICE Quality Standards in Acute Heart Failure (2015) and Chronic Heart Failure (2018). The post holder will work strategically with senior colleagues across the health community to ensure that these standards are delivered at Nottingham University Hospitals NHS Trust.

The post holder will be responsible for co-ordinating both inpatient and outpatient services for patients with heart failure and providing clinical advice/ support to nursing staff and other health care professionals.

Essential criteria

NMC registration
Nursing related Degree

Desirable criteria

Possesses non-medical prescribing qualification
Completed or presently undertaking a Masters

Experience

Evidence of experience in Cardiology nursing
Experience in managing patients with Heart Failure

Analytical and Judgement skills

Essential criteria

Evidence of the ability to lead and motivate change in practice
Evidence to prioritise actions and to provide advice to the multi professional team

Demonstrate professional autonomy and the ability to work at a higher level, exhibiting effective decision making skills.

Salary: The quoted salary will be on a pro rata basis for part time workers.

We limit this to one responsibility to allow for focused time on that topic, rather than having your time stretched too thinly. When taking on an additional responsibility, your case volume is reduced, making time available for this focused time.

For Senior Assessors, additional responsibility options include supporting more complex cases, ensuring quality assurance, providing training, facilitating family engagement, and contributing to pathway development.
