Job overview

We are seeking a highly motivated Occupational Therapist to join our evolving Integrated Community Stroke Service at Sherwood Forest Hospitals NHS Foundation Trust. This is an exciting opportunity to work within a high‑profile, specialist community service that makes a real difference to patients’ lives.

This is a development post, starting at Band 5 with clear progression to Band 6 following successful completion of preceptorship and role‑specific competencies. You will be well supported through structured learning, supervision and opportunities to develop your clinical skills in stroke rehabilitation.

Established in 2009 as the Stroke Early Supported Discharge team, the service expanded in April 2023 to become an Integrated Community Stroke Service, delivering specialist, community‑based stroke rehabilitation across Mansfield, Ashfield, Newark and Sherwood.

The service supports patients in their transition from hospital to home, promoting recovery, independence and quality of life during the first six months post‑discharge. Working in patients’ own environments enables truly person‑centred rehabilitation and strong therapeutic relationships.

You will join a supportive multidisciplinary team with a strong commitment to clinical excellence and continuous improvement. We are looking for an Occupational Therapist with a genuine interest in stroke care, who is motivated by challenge and keen to contribute to both service development and their own professional growth.

  • Degree in Occupational Therapy, BSc(Hons) or Graduate Diploma in Occupational Therapy
  • HCPC State Registration
Desirable criteria
  • Membership to Royal College of Occupational Therapists

Knowledge

Essential criteria
  • Ability to assess, plan and evaluate treatment.
  • Knowledge of equipment/adaptations and all aspects of ADL
  • Time management & prioritisation skills
  • Competent IT skills
  • Self motivated and able to work as an effective team member.
  • Effective communicate skills
  • Ability to manage self.
  • Flexible and positive approach.
  • Organisational skills and use of initiative
  • Clinical reasoning and reflective practitioner
Desirable criteria
  • Knowledge of local & National priorities
  • Pathology of the older person/Stroke
  • Knowledge of Risk assessment

Contractual Requirements

Essential criteria
  • All appointments are subject to satisfactory medical clearance and the ability to cope with the demands of the role
  • Able to cover 8am -6pm including weekends and bank holidays to meet the needs of the service
  • Full driving licence
  • Car driver – daily access to own transport

Experience

Essential criteria
  • Broad experience in general occupational therapy application
  • Continued Professional Development
Desirable criteria
  • Experience with stroke patients
  • Experience of community working
