An exciting opportunity has arisen for an enthusiastic individual to join our community service, this post will focus on ‘high risk’ frail patients supporting early detection of any medical, social or psychological crisis.
This post will work with primary care and enhanced community services they will carry a complex caseload. As this is a new role they will support the development of Frailty pathways across primary and enhanced community services
As a highly skilled, senior AHP practitioner the successful candidate will have expert knowledge in the management of frailty to prevent avoidable hospital admissions and support timely discharge home. They will work collaboratively with primary, community and secondary care services providing clinical leadership to teams delivering rehabilitation and developing pathways.
Main duties of the job
To undertake autonomous complex assessments and to evaluate and make clinical decisions regarding patient care for those requiring enhanced community service. This will include completing comprehensive geriatric assessment to frail individuals in their home environment and developing the role of trusted assessor to achieve a positive outcome for the patients.
Focusing on preventative strategies this role would target /identify those people at home in the community with complex long-term conditions, where there is often a mix of physical and mental health distress who are escalating in their use of services. The practitioners would review, assess and support the development of self-management and social management strategies alongside medical management to reduce on-going health and care needs.
The candidate would have excellent communication skills and liaise with the multidisciplinary team including the secondary care GPs, patient relatives and third sector partners.
The candidate would have evidence of post registration experience and would need to demonstrate a flexible approach as this is a new post and the role will evolve and may change depending on the demands of the service.

This post requires the successful applicant to be able to work in in the community and therefore the ability to travel between sites in a timely manner is essential.

Qualifications and/or Knowledge
Essential criteria
- Diploma/ Degree in Occupational Therapy / Physiotherapy
- HCPC registration
- Advanced accredited training at master’s level or equivalent
- Accredited post graduate specialist courses/ training relevant to role
Desirable criteria
- MSc within relevant specialty
- Relevant leadership courses/ qualifications
- Post-graduate qualification up to post grad diploma level in relevant area
- Member of the Royal College of Occupational Therapy/Chartered Society of Physiotherapy

- Significant postgraduate experience relevant to specialty
